Many questions have been raised about the program since DOD began vaccinating its 2.4 million active duty and reserve members in 1998. A major concern has been the program's effect on the National Guard and Air Force Reserve's retention of trained and experienced personnel. A questionnaire sent to 1,253 randomly selected Guard and Reserve pilots and others revealed that the anthrax immunization was a key reason these individuals left or otherwise changed their military status.
