H.R. 4856--A bill to amend title XVIII of the Social Security Act to 
    require physicians and others requesting payment on the basis of 
    an assignment under the medicare program to provide a copy of the 
    completed request for payment form to the patient; jointly, to the 
    Committees on Ways and Means; Energy and Commerce.
