FOREWORD
1.	This manual outlines uniform procedures which will be followed by military police in towns and cities, and at posts, camps, and stations, in the use of forms which have been standardized. This will reduce paper work and the number of forms produced locally. The procedures set forth herein for the use of WD AGO forms have been developed and tested extensively in the field. In some cases they represent major changes in existing methods. These forms will be requisitioned from adjutant general depots.
2.	Forms illustrated in this manual bearing WD AGO R designation numbers are intended as guides and may be produced locally. The suggested specifications are indicated below each sample form.
3.	Military police organizations maintain then-own administrative records and files to meet local requirements. References to files and cross-indexes contained herein are for the sole purpose of contributing to simplicity and maximum operating efficiency. Although this manual does not deal with statistical or periodic reports required by higher headquarters, or reports maintained locally for miscellaneous purposes, the number of such reports should be held to a minimum consistent with efficient operation.
4.	Changes to this manual will be supplied on a page basis, and will be published as required. As change pages are received they will be inserted in their proper places, and the replaced pages destroyed.
5.	Each page of the manual bears a date in its upper inside corner. This date is the date of publication. Pages which represent changes will carry the date and number of the change.
6.	Pages are numbered consecutively throughout the book. If new pages are added within the book the added pages will carry alphabetical suffixes A, B, C, and so on. For example, if a page is added between 9 and 10, the page will be numbered 9A. A second additional page in the same place would be numbered 9B, and so on.
7.	In order to show the relationship between the various forms, the original case numbers, dates, and record of arrest numbers used in the illustrations will be retained in changes to the manual, even though later editions of the forms are published.
8.	Care will be exercised to insure that military police reports and forms are accurately and impartially prepared. In reporting alleged crimes, the fundamentals of report writing as described in chapter 14, FM 19-20, will be observed.
9.	For instructions pertaining to the preparation of forms necessary in the administration of guardhouses and confinement facilities, see TM 12-510.
10.	Recommendations for improvements in forms and procedures should be transmitted through channels to The Provost Marshal General, Washington 25, D. C.

SECTION I
MILITARY POLICE REPORTS AND RECORDS
1.	Essentials of Record System
a.	Records maintained by military police should be held to a minimum, both in number and volume, consistent with efficient operation. Information recorded on the WD AGO forms discussed in this chapter is basic in any system of military police records. The number of copies of any record or
report made will depend upon local requirements, but will be sufficient to comply with War Department directives.
b.	Two entirely distinct and unrelated series of numbers are used on various military police forms. These are “Record of Arrest” and “Case” numbers. The numbers of each series are assigned

consecutively. Each series usually runs for a calendar year.
(1)	A Record of Arrest Number is assigned to each person arrested at the time he is booked. The number is placed in the first column of the Record of Arrests, WD AGO Form 19-76. Whenever a Record of Arrest Number is used on another form it should be copied from the Record of Arrests.
(2)	A Case Number is assigned to each Report of Arrest, Delinquency, or Incident, WD AGO Form 19-71. The numbers are assigned consecutively regardless of the nature of the report. Whenever a Case Number is used on another form it should be copied from the original or a file copy of the Report of Arrest, Delinquency, or Incident.
c.	A copy of each Report of Arrest, Delinquency, or Incident, WD AGO Form 19-71, with all other records pertaining to the case, other than those in bound volumes, should be filed together by Case Number. Filing by Case Number makes the file chronological.
d.	An alphabetical card index to the case file should be maintained as a cross-reference. Figure 18 presents a suggested form for cards in this index.
e.	An essential part of the record system is a journal, in which should be entered information
and orders received, and action taken, which it is desired to preserve for future reference and which is not recorded in the Record of Arrests (WD AGO Form 19-76) book, or in other permanent records. Figure 1 presents a suggested journal page form.
2. Journal WD AGO Form R-5195
a.	The journal (fig. 1) contains entries on information and orders received, and action taken, which are desired for future reference, and which are not recorded in other permanent records maintained by the military police headquarters.
b.	It is suggested that the journal be maintained in a loose-leaf notebook on unruled paper. Entries should be typed or recorded in ink.
c.	The type of entries to be made will be determined by the commanding officer. They normally include complaints and other information received by telephone or from office visitors. Items recorded under “Action Taken” may relate to entries in the adjoining “Information or Order Received” column, or may be independent thereof. A widely-accepted practice of setting off the different items is to draw a fine across the page under each entry.

3.	Radio Log, WD AGO Form R-5196
a.	When military police operate a radio transmitter it may be desirable for operators at the central station to log their calls on a form such as that shown in figure 2.
b.	“From” and “to” in this suggested log form refer to stations, not to individuals. The name of the person transmitting the message is entered under “name.”
c.	For brevity and security, messages may be sent in code, in which case the code message is entered.

a.	Record of Arrests books should be numbered serially, and the date of the last entry filled in after the word “to” on the cover. (See fig. 3@.)
b.	All persons detained are booked in the Record of Arrests on arrival at the military police jail. (See fig. 3@.)
c.	Dates should normally be entered at the beginning of each day. In the case of a station operating only part of the time, for instance from 1800 to 0200, it may be preferable to make date entries for each tour of duty, 1-2 June 1945, 2-3 June 1945, etc.
d.	If desired, incidents may also be noted in the Record of Arrests.

a.	When a detained person is booked he is searched, and money and other property of intrinsic value, or which might be used as weapons, is taken for safekeeping during his confinement.
b.	Instructions for use of the Prisoner Property Record, WD AGO Form 19-77, appear on the inside cover thereof. (See fig. 4®.)
c.	When a Prisoner Property Record book is closed out the cover (fig. 4®) should be completed by entering the date of the last receipt after
“to.” Books are numbered serially in order of use.
6.	Property Label WD AGO Form R-5197
a.	This tag (fig. 5) is used for labeling found and seized articles. It is intended primarily for bulky objects.
b.	It is used to identify property which is found, which is seized as evidence, or which is received from prisoners or other persons for safekeeping. The military policeman receiving the property will make the appropriate entries on the tag.

INSTRUCTIONS FOR USE OF FORM
1. This form will be used to report all arrests, delinquencies, or incidents.
Full particulars in the case, including names and addresses of all persons involved and all witnesses, including their statements if available, will be forwarded with this report It is of the UTMOST IMPORTANCE that the interests of the United States be protected in all cases where personal injury is a factor and in all cases wherein Government vehicles or equipment are involved.
Reports of disciplinary action taken will or will not be requested in accordance with existing instructions.
Forwarding of the report will conform to prescribed policies.

a.	Figures 9 and 10 are suggested models for sworn statements by accused persons, and for affidavits by other persons having knowledge of matters normally investigated by military police.
b.	Sworn statements and affidavits must be acknowledged before a notary public, an adjutant, a summary court, or other person authorized to administer oaths. Ordinarily, an officer who has been appointed as a summary court will be available to take sworn statements and affidavits.
11.	Descriptive List of Absentee, WD AGO Form 45
a.	Provost marshals should arrange to have copies of WD AGO Form 45 (fig. 11) forwarded to their offices by units stationed in their areas.
b.	These forms should be filed alphabetically, and kept available to desk sergeants at all times. Copies should be posted for the information of all military policemen.
c.	Upon receipt of a WD AGO Form 46, Report of Return of Absentee to the United States Army (fig. 12), in a given case, or upon arrest of the absentee, the corresponding WD AGO Form 45, should be removed from the active file and the bulletin board.

Any civil officer having authority to arrest offenders may arrest a man absent without leave from the military service of the United States when authorized by any military officer, and deliver him into the custody of the military authorities of the United States. Receipt of* this form or a communication (oral or written) from an Army officer stating that a named person is absent without leave is authority for arrest and detention.

Provost marshals should arrange to have copies of WD AGO Form 46 (fig. 12), sent to them by units from which they receive copies of WD AGO Form 45, Descriptive List of Absentee Wanted by the United States Army. (See fig. 11.)
13.	Provisional Military Police Pass, WD
AGO Form 19-75 (See FM 19-10 and TM 19-275)
a.	WD AGO Form 19-75 passes (fig. 13) are
issued to absentee enlisted personnel directed to return unguarded to their organizations.
b.	Provisional military police passes are usually issued by desk sergeants, and by military police on trains. They should be made out either on a typewriter or in ink.
c.	Soldiers to whom these passes are given sign the acknowledgment on the back of the form.
d.	The number of copies of these passes will be governed by local requirements, but will usually include one to the soldier, one to his commanding officer, and one for file.

21.	General
Experience has demonstrated that most routine application forms are unnecessary when adequate information is given on registration and permit forms, with one or more copies retained for alphabetical or numerical files. References to this practice appear in the notes on the applicable forms presented in this chapter.
A numerical record of registrations and permits may be kept on a form such as that shown in figure 21, in lieu of the more bulky card index.
22.	Numerical Record of Registrations and Permits, WD AGO Form R-5205
a.	The form in figure 21 is suggested for use in keeping numerical records of registrations and permits. When it is used, a separate record or list should be maintained for each type of registration or permit.
b.	The use of Acco fasteners is a convenient means of binding together the sheets of each set of papers.

Persons permitted to visit restricted areas of posts should be issued passes when they enter. Each pass should be collected when the holder leaves the post, and checked against the duplicate retained at the place of issue. The amount of information recorded on the pass will be determined by local requirements.

e.	Passes should be numbered. This may be done by placing the number on the title board at the time the individual is photographed.
/. While passes will generally be issued by the provost marshal, they usually are issued at the request of branch chiefs or other responsible personnel of the post, who may be required to sign the card pass before it is authenticated by the provost marshal.
g. If either a numerical or an alphabetical index of card passes is desired, the passes may be prepared in duplicate or triplicate to meet requirements. Cards of a similar nature may be used in making an index for button passes.

29. Personal Identification Fingerprint Card
a.	Use of the Federal Bureau of Investigation applicant card pictured in figure 32 is suggested when it is desired to submit fingerprints of job applicants to the FBI for checking against its files. Other acceptable forms include the FBI’s earlier “National Defense Program” fingerprint card, upon which substantially the same information is recorded.
b.	Normal procedure is to submit one copy of the completed form to the Identification Division, Federal Bureau of Investigation, Washington 25, D. C.
c.	Whether a duplicate set of fingerprints should be taken will be governed by local requirements.
