The ICR describes the nature of the information collection and its expected cost and burden. SUPPLEMENTARY INFORMATION: Office of the Secretary Title: Indoor Air Quality Medical Questionnaire. OMB Control Number: 2105-0533. Type of Request: New Collection. Affected Entities: 5,500 Occupants of the U.S. Department of Transportation workers in the Nassif Building. Abstract: The Department of Transportation (DOT) is announcing a 3- year voluntary health questionnaire to conduct surveys to provide medical evaluations of DOT workers in the Nassif Building. Participation is entirely voluntary. Health surveys of the Nassif Building occupants will be conducted to help determine the role that the building conditions play in employees [[Page 4828]] health. In several weeks, a survey will be conducted to establish a baseline of information. The same survey will be conducted again after the cleaning and repair of the building is complete to further identify the link between employees' symptoms and building conditions. The results of the survey will provide updated data on the status of employees' health as it relates to the Nassif Building. Estimated Total Burden on Respondents: 1,500 hours.
