    Title of Forms: Customer Satisfaction Form.
    Need and Use: The National and Community Service Trust Act of 1993 
(Pub. L. 103-82) requires the Corporation for National and Community 
Service to evaluate its programs on a regular basis. This information 
is required for program management, planning, and required record 
keeping.
    Type of request: Submission of a new collection.
    Respondent's Obligation to Reply: Voluntary.
    Frequency of Collection: One time only.
    Estimated Number of Responses: 10,110.
    Average Burden Hours Per Response: 0.042 Hours.
