<distribution-code display="yes">II</distribution-code><congress>119th CONGRESS</congress><session>2d Session</session><legis-num>S. 4225</legis-num><current-chamber>IN THE SENATE OF THE UNITED STATES</current-chamber><action><action-date date="20260326">March 26, 2026</action-date><action-desc><sponsor name-id="S370">Mr. Booker</sponsor> (for himself and <cosponsor name-id="S288">Ms. Murkowski</cosponsor>) introduced the following bill; which was read twice and referred to the <committee-name committee-id="SSVA00">Committee on Veterans' Affairs</committee-name></action-desc></action><legis-type>A BILL</legis-type><official-title>To require the Secretary of Veterans Affairs to establish a pilot program to furnish doula services to veterans.</official-title></form><legis-body style="OLC" display-enacting-clause="yes-display-enacting-clause" id="H7CC809153E194466BE516F6F2CFD5E5F"><section section-type="section-one" id="HB851C330C14A4ADF8CEC796537B2BDE8"><enum>1.</enum><header>Short title</header><text display-inline="no-display-inline">This Act may be cited as the <quote><short-title>Delivering Optimally Urgent Labor Access for Veterans Affairs Act of 2026</short-title></quote> or the <quote><short-title>DOULA for VA Act of 2026</short-title></quote>.</text></section><section id="H584EB6EEDBA243DCA0EB22C17B3C5F1E"><enum>2.</enum><header>Pilot program on doula support for veterans</header><subsection id="H5F781ACD1A2A42F785A5A689CBF49960"><enum>(a)</enum><header>Findings</header><text>Congress finds the following:</text><paragraph id="H3A8237FA56DF412F8394FF8225967792"><enum>(1)</enum><text>There are approximately 2,100,000 women within the veteran population in the United States, the fastest growing demographic group of veterans.</text></paragraph><paragraph id="H5038FB6A9A6F48EFB204D0E8798EE290"><enum>(2)</enum><text>By 2020, the proportion of all women veterans using health care from the Department of Veterans Affairs grew to 28 percent from 17 percent in 2010.</text></paragraph><paragraph id="ide3ad1e3412d24cd9857f43497164dcd2"><enum>(3)</enum><text>During the period of 2010 through 2020, the number of women veterans using health care from the Department nearly doubled, from 316,961 to 556,135.</text></paragraph><paragraph commented="no" display-inline="no-display-inline" id="id6d82e8661d11450fbf4d1b54071f9e30"><enum>(4)</enum><text display-inline="yes-display-inline">In 2025, there are more than 600,000 women receiving health care from the Department.</text></paragraph><paragraph id="HD3016CCCE8D649498312E839A287C113"><enum>(5)</enum><text>Although prenatal care and delivery is not provided in facilities of the Department, pregnant women seeking care from the Department for other conditions may also need emergency care and require coordination of services through the Veterans Community Care Program under section 1703 of title 38, United States Code.</text></paragraph><paragraph id="HD8BEC2F688DF497BBB33738C71DCEF82"><enum>(6)</enum><text>The number of women veteran patients with obstetric deliveries paid for by the Department increased to more than 5,500 in fiscal year 2019, triple the number of such patients in fiscal year 2009, and 22 times the number from fiscal year 1999.</text></paragraph><paragraph id="H4CF1C6E8A8244030B475F9CA4C1418E8"> <enum>(7)</enum> <text>Notably, a quarter of the deliveries specified in paragraph (5) from fiscal year 2019 were among veterans of advanced maternal age (35 years old or older) and a quarter were among Black or African-American veterans, both groups known to experience higher risk of severe maternal morbidity.</text>
 </paragraph><paragraph id="H9CC1280BC5FC4A2C8DD9BC509B812ECA"><enum>(8)</enum><text>A study in 2010 found that veterans returning from Operation Enduring Freedom and Operation Iraqi Freedom who experienced pregnancy were twice as likely to have a diagnosis of depression, anxiety, posttraumatic stress disorder, bipolar disorder, or schizophrenia as those who had not experienced a pregnancy.</text></paragraph><paragraph id="H880B145405664C0CAD1A4D1B2508CFF0"><enum>(9)</enum><text>The number of women veterans of reproductive age seeking care from the Department continues to grow (more than 228,000 as of fiscal year 2020).</text></paragraph></subsection><subsection id="HB6A0E809E2A5498E8F66E1D806633A39"><enum>(b)</enum><header>Program</header><paragraph id="H26740FBD363B4233A82BD9ADB4F15D3D"><enum>(1)</enum><header>In general</header><text>Not later than one year after the date of the enactment of this Act, the Secretary of Veterans Affairs shall establish a pilot program to furnish doula services to covered veterans through eligible entities by expanding the Whole Health model of the Department of Veterans Affairs, or successor model, to measure the impact that doula support services have on birth and mental health outcomes of pregnant veterans (in this section referred to as the <quote>pilot program</quote>).</text></paragraph><paragraph id="HF96E5A748943453DA6FCB19B64679B03"><enum>(2)</enum><header>Consideration</header><text>In carrying out the pilot program, the Secretary shall consider all types of doulas, including traditional and community-based doulas.</text></paragraph><paragraph id="H3DD0DAEC614C416696E9CF6A58C5DA2D"><enum>(3)</enum><header>Consultation</header><text>In designing and implementing the pilot program, the Secretary shall consult with stakeholders, including—</text><subparagraph id="HBF875C5D4B6545DA8E7AE3E164E73756"><enum>(A)</enum><text>organizations representing veterans, including veterans that are disproportionately impacted by poor maternal health outcomes;</text></subparagraph><subparagraph id="H8ABBED44E1164E29B0C23E47ED9320BA"><enum>(B)</enum><text>community-based health care professionals, including doulas, and other stakeholders; and</text></subparagraph><subparagraph id="HC96B8D68DE254BE59A05ADA1B9E56948"><enum>(C)</enum><text>experts in promoting health equity and combating racial bias in health care settings.</text></subparagraph></paragraph><paragraph id="H9452D51A49FB4DBDBB19FCC060722705"><enum>(4)</enum><header>Goals</header><text>The goals of the pilot program are the following:</text><subparagraph id="HE87CDF7F471A40668BFE6E8036328740"><enum>(A)</enum><text>To improve—</text><clause id="HF0E1002948AD4FBB848AA8F3B4C4F7E8"><enum>(i)</enum><text>maternal, mental health, and infant care outcomes;</text></clause><clause id="H4DE5EE0316434BC7ADDF52B2E7A31D92"><enum>(ii)</enum><text>integration of doula support services into the Whole Health model of the Department, or successor model; and</text></clause><clause id="H8A10C6830735455C9E511ACDEA46D4B3"><enum>(iii)</enum><text>the experience of women receiving maternity care from the Department, including by increasing the ability of a woman to develop and follow her own birthing plan.</text></clause></subparagraph><subparagraph id="H195090C92B3E4229928E79736E1AB5D2"><enum>(B)</enum><text>To reengage veterans with the Department after giving birth.</text></subparagraph></paragraph></subsection><subsection id="HB57CB720F1924453A956EC4BC5837EAD"><enum>(c)</enum><header>Locations</header><text>The Secretary shall carry out the pilot program in—</text><paragraph id="HF7A3C79891704828894846246AE32214"><enum>(1)</enum><text>the three Veterans Integrated Service Networks of the Department that have the highest percentage of female veterans enrolled in the patient enrollment system of the Department established and operated under section 1705(a) of title 38, United States Code, compared to the total number of enrolled veterans in such Network;</text></paragraph><paragraph id="H387B23966CB3404B9F7E21102E762780"><enum>(2)</enum><text>the three Veterans Integrated Service Networks that have the lowest percentage of female veterans enrolled in the patient enrollment system compared to the total number of enrolled veterans in such Network; and</text></paragraph><paragraph commented="no" id="id63d55b26c8374bd39f2b50ad4cb372f8"><enum>(3)</enum><text>at least one Veterans Integrated Services Network—</text><subparagraph commented="no" display-inline="no-display-inline" id="id92aafaa1fefc4591a56e566b83d7cb07"><enum>(A)</enum><text display-inline="yes-display-inline">located in or serving a Frontier State (as defined in section 1886(d)(3)(E)(iii)(II) of the Social Security Act (<external-xref legal-doc="usc" parsable-cite="usc/42/1395ww">42 U.S.C. 1395ww(d)(3)(E)(iii)(II)</external-xref>)) where more than <fraction>1/3</fraction> of the population lives in frontier land; and</text></subparagraph><subparagraph commented="no" display-inline="no-display-inline" id="id6195d0b5217041fdb7066da9bb931d5d"><enum>(B)</enum><text display-inline="yes-display-inline">serving populations experiencing higher average risk and prevalence for maternal mental health disorders, including American Indian or Alaska Native veterans.</text></subparagraph></paragraph></subsection><subsection id="H0C9756F2FB0C4F4A8BE85B2030CE049D"><enum>(d)</enum><header>Open participation</header><text>The Secretary shall allow any eligible entity or covered veteran interested in participating in the pilot program to participate in the pilot program.</text></subsection><subsection id="H49BEE91973274743A5009A88C9906C6E"><enum>(e)</enum><header>Services provided</header><paragraph id="HEDF07A410D0B4FAD8BD187FED3FF19EC"><enum>(1)</enum><header>In general</header><text>Under the pilot program, a covered veteran shall receive not more than 10 sessions of care from a doula under the Whole Health model of the Department, or successor model, under which a doula works as an advocate for the veteran alongside the medical team for the veteran.</text></paragraph><paragraph id="HD167363D2E394758942F7A076A47AD7E"><enum>(2)</enum><header>Sessions</header><text>Sessions covered under paragraph (1) shall be as follows:</text><subparagraph id="H1D78DE750486481DAA8E06ED892B6A33"><enum>(A)</enum><text>Three or four sessions before labor and delivery.</text></subparagraph><subparagraph id="HA105FDCEE8254C4881AF2A5AAF20DDDD"><enum>(B)</enum><text>One session during labor and delivery.</text></subparagraph><subparagraph id="HFE297739DE02485480362FB4274988E1"><enum>(C)</enum><text>Three or four sessions after post-partum, which may be conducted via the mobile application for VA Video Connect.</text></subparagraph></paragraph></subsection><subsection id="H0AFCC60CA26D46909231DB01072DBE89"><enum>(f)</enum><header>Administration of pilot program</header><paragraph id="H2666DEE3792B4488AE89D28CBB743F4D"><enum>(1)</enum><header>In general</header><text display-inline="yes-display-inline">The Office of Women’s Health of the Department of Veterans Affairs, or successor office (in this section referred to as the <quote>Office</quote>), shall—</text><subparagraph id="H3936203AE7DE44F5ADE2F151BA16AE76"><enum>(A)</enum><text>coordinate services and activities under the pilot program;</text></subparagraph><subparagraph id="H2FC076B6B8134872929FD0375B9074EE"><enum>(B)</enum><text>oversee the administration of the pilot program; and</text></subparagraph><subparagraph id="H576BD47AEA094C63ADB22FFDDF989919"><enum>(C)</enum><text>conduct onsite assessments of medical facilities of the Department that are participating in the pilot program.</text></subparagraph></paragraph><paragraph id="H92C26C9E7EDC4D7D9D580AF9D9BEB9C0"> <enum>(2)</enum> <header>Guidelines for veteran-specific care</header> <text>The Office shall establish guidelines under the pilot program for training doulas on military sexual trauma and posttraumatic stress disorder.</text>
 </paragraph><paragraph id="H55596FCA1EDA45FB8B2C0EB0D3E84C5A"><enum>(3)</enum><header>Amounts for care</header><text display-inline="yes-display-inline">The Office may recommend to the Secretary appropriate payment amounts for care and services provided under the pilot program, which shall not exceed $3,500 per doula per veteran.</text></paragraph></subsection><subsection id="HD3391B34FBAA47CE8FA8110F608C8B6D"><enum>(g)</enum><header>Doula Service Coordinator</header><paragraph id="H705FA81F7CDF4B63972B4B3BB439E28A"><enum>(1)</enum><header>In general</header><text display-inline="yes-display-inline">The Secretary, in consultation with the Office, shall establish a Doula Service Coordinator within the functions of the Maternity Care Coordinator at each medical facility of the Department that is participating in the pilot program.</text></paragraph><paragraph id="H879CD50146C64CAB9BEFCF0910892873"><enum>(2)</enum><header>Duties</header><text>A Doula Service Coordinator established under paragraph (1) at a medical facility shall be responsible for—</text><subparagraph id="HECB7D07141B940D69F04B237518D3A5B"><enum>(A)</enum><text>working with eligible entities, doulas, and covered veterans participating in the pilot program; and</text></subparagraph><subparagraph id="H673CEBBE1E5B4DADBA2B851F8899445E"><enum>(B)</enum><text>managing payment between eligible entities and the Department under the pilot program.</text></subparagraph></paragraph><paragraph id="HE57F3D5B133A47B7B929EDB2AD62BBC1"><enum>(3)</enum><header>Tracking of information</header><text>A doula providing services under the pilot program shall report to the applicable Doula Service Coordinator after each session conducted under the pilot program.</text></paragraph><paragraph id="HC23DDA81E0C04759ADE98641D3652301"><enum>(4)</enum><header>Coordination with women's program manager</header><text>A Doula Service Coordinator for a medical facility of the Department shall coordinate with the women's program manager for that facility in carrying out the duties of the Doula Service Coordinator under the pilot program.</text></paragraph></subsection><subsection id="H8C7D1C4ADA8C480899B5544F9A69B1F8"><enum>(h)</enum><header>Term of pilot program</header><text>The Secretary shall conduct the pilot program for a period of five years.</text></subsection><subsection id="H39F0D20D321545719710086E3FD3BFF4"><enum>(i)</enum><header>Technical assistance</header><text>The Secretary shall establish a process to provide technical assistance to eligible entities and doulas participating in the pilot program.</text></subsection><subsection id="H106CFE144FF848D8BF9381377D7F6B51"><enum>(j)</enum><header>Report</header><paragraph id="HE26C49D3AAC241ECA797795B76C10D9B"><enum>(1)</enum><header>In general</header><text>Not later than one year after the date of the enactment of this Act, and annually thereafter for each year in which the pilot program is carried out, the Secretary shall submit to the Committee on Veterans’ Affairs of the Senate and the Committee on Veterans’ Affairs of the House of Representatives a report on the pilot program.</text></paragraph><paragraph id="HE6ADD1D74B814D0BB4A82BA5094D8BD6"><enum>(2)</enum><header>Final report</header><text>As part of the final report submitted under paragraph (1), the Secretary shall include recommendations on whether the model studied in the pilot program should be continued or more widely adopted by the Department.</text></paragraph></subsection><subsection id="HCAC9045B506A449EBE3B19C8BAEEC4A0"><enum>(k)</enum><header>Authorization of appropriations</header><text>There are authorized to be appropriated to the Secretary, for each of fiscal years 2027 through 2032, such sums as may be necessary to carry out this section.</text></subsection><subsection id="H4A33D0DE451E44A8B0060DE6C9C73FC3"><enum>(l)</enum><header>Definitions</header><text>In this section:</text><paragraph id="H9A688B60CBC243AB8650AD7B17B7F8B6"><enum>(1)</enum><header>Covered veteran</header><text>The term <term>covered veteran</term> means a pregnant veteran or a formerly pregnant veteran (with respect to sessions post-partum) who is enrolled in the patient enrollment system of the Department of Veterans Affairs established and operated under section 1705(a) of title 38, United States Code.</text></paragraph><paragraph id="HC7DBA16CBE8C48BD94313892183F9F2B"><enum>(2)</enum><header>Eligible entity</header><text>The term <term>eligible entity</term> means an entity that provides medically accurate, comprehensive maternity services to covered veterans under the laws administered by the Secretary, including under the Veterans Community Care Program under section 1703 of title 38, United States Code.</text></paragraph><paragraph id="H04EEED166C624398A2FBBBE8918577C9"><enum>(3)</enum><header>VA Video Connect</header><text>The term <term>VA Video Connect</term> means the program of the Department of Veterans Affairs to connect veterans with their health care team from anywhere, using encryption to ensure a secure and private session.</text></paragraph></subsection></section></legis-body></bill>
