which was read twice and referred to the <committee-name committee-id="SSHR00">Committee on Health, Education, Labor, and Pensions</committee-name></action-desc></action><legis-type>A BILL</legis-type><official-title>To authorize the Director of the Centers for Disease Control and Prevention to carry out a Social Determinants of Health Program, and for other purposes.</official-title></form><legis-body style="OLC" display-enacting-clause="yes-display-enacting-clause" id="HBFEB440EF1A248C68412ACBE4C55469A"><section section-type="section-one" id="H4C31668BF9A144E09FCDD90025B70210"><enum>1.</enum><header>Short title</header><text display-inline="no-display-inline">This Act may be cited as the <quote><short-title>Improving Social Determinants of Health Act of 2024</short-title></quote>.</text></section><section id="H837D443728CE4D149042B5EA6B5891D9"><enum>2.</enum><header>Social Determinants of Health Program</header><subsection id="H5535BCB485D74672AB9FDCDF827123F9"><enum>(a)</enum><header>Program</header><text>To the extent and in the amounts made available in advance in appropriations Acts, the Director of the Centers for Disease Control and Prevention (in this Act referred to as the <quote>Director</quote>) shall carry out a program, to be known as the Social Determinants of Health Program (in this Act referred to as the <quote>Program</quote>), to achieve the following goals:</text><paragraph id="H53B21E5B075E40D485EC4666ADD10484"><enum>(1)</enum><text display-inline="yes-display-inline">Improve health outcomes and reduce health inequities by coordinating social determinants of health activities across the Centers for Disease Control and Prevention.</text></paragraph><paragraph id="HC95E0AA9F09D431996085805B097DA34"><enum>(2)</enum><text display-inline="yes-display-inline">Improve the capacity of public health agencies and community organizations to address social determinants of health in communities.</text></paragraph></subsection><subsection id="H4679AE6E79C649A9AE68B39E35AAAFD7"><enum>(b)</enum><header>Activities</header><text>To achieve the goals listed in subsection (a), the Director shall carry out activities including the following:</text><paragraph id="HC90327CBCFA34762BE1A6875ADB05151"><enum>(1)</enum><text>Coordinating across the Centers for Disease Control and Prevention to ensure that relevant programs consider and incorporate social determinants of health in grant awards and other activities.</text></paragraph><paragraph commented="no" id="H895F4DF74EB34041A495680FB67BAB0B"><enum>(2)</enum><text display-inline="yes-display-inline">Awarding grants under section 3 to State, local, territorial, and Tribal health agencies and organizations, and to other eligible entities, to address social determinants of health in target communities. </text></paragraph><paragraph commented="no" id="H810915688FA745FF85224A10B940E73B"><enum>(3)</enum><text display-inline="yes-display-inline">Awarding grants under section 4 to nonprofit organizations and public or other nonprofit institutions of higher education—</text><subparagraph commented="no" id="H151B495A3A90491F895EC3B63A905F71"><enum>(A)</enum><text display-inline="yes-display-inline">to conduct research on best practices to improve social determinants of health;</text></subparagraph><subparagraph commented="no" id="HB73AFDF7E0FB45A5B25FAF231A6FC31C"><enum>(B)</enum><text>to provide technical assistance, training, and evaluation assistance to grantees under section 3; and</text></subparagraph><subparagraph commented="no" id="H67EEA0E921004CF18ABBF6276F845A69"><enum>(C)</enum><text>to disseminate best practices to grantees under section 3.</text></subparagraph></paragraph><paragraph id="H679D411A5E244966807BFB5A374B553E"><enum>(4)</enum><text>Coordinating, supporting, and aligning activities of the Centers for Disease Control and Prevention related to social determinants of health with activities of other Federal agencies related to social determinants of health, including such activities of agencies in the Department of Health and Human Services such as the Centers for Medicare &amp; Medicaid Services.</text></paragraph><paragraph id="H4839104DEC9346B8BEA044397ADDB5EE"><enum>(5)</enum><text>Collecting and analyzing data related to the social determinants of health.</text></paragraph></subsection></section><section id="HB14A07F6132E4B53AEC1EF6569C2E94E"><enum>3.</enum><header>Grants To address social determinants of health</header><subsection id="H47A6A1FF9DB14A0398090A56994599F3"><enum>(a)</enum><header>In general</header><text>The Director, as part of the Program, shall award grants to eligible entities to address social determinants of health in their communities.</text></subsection><subsection id="H68BD4AAA3C7A4B3D953DCB961AD12EF0"><enum>(b)</enum><header>Eligibility</header><text display-inline="yes-display-inline">To be eligible to apply for a grant under this section, an entity shall be—</text><paragraph id="HD6FFF9B517384B0FA0D09BB4E5193646"><enum>(1)</enum><text>a State, local, territorial, or Tribal health agency or organization;</text></paragraph><paragraph id="H935ABCA90D544AAFBF31D8D1540C60EB"><enum>(2)</enum><text>a qualified nongovernmental entity, as defined by the Director; or</text></paragraph><paragraph id="H9D5C22D6960E4A3CA299866ED0E8DDC9"><enum>(3)</enum><text>a consortium of entities that includes a State, local, territorial, or Tribal health agency or organization.</text></paragraph></subsection><subsection id="H6FA04139D44F4E75AB02BE60A86DAF7E"><enum>(c)</enum><header>Use of funds</header><paragraph id="HD4322F47B4A443B3A497C41284122624"><enum>(1)</enum><header>In general</header><text>A grant under this section shall be used to address social determinants of health in a target community by designing, implementing, and evaluating innovative, evidence-based, cross-sector strategies.</text></paragraph><paragraph id="H2D130B71889B4773940AB3D27A6E5187"><enum>(2)</enum><header>Target community</header><text>For purposes of this section, a target community shall be a State, county, city, or other municipality.</text></paragraph></subsection><subsection id="H2C641D9A1E714D8BBAC85166D6C26BE6"><enum>(d)</enum><header>Priority</header><text display-inline="yes-display-inline">In awarding grants under this section, the Director shall prioritize applicants proposing to serve target communities with significant unmet health and social needs, as defined by the Director. </text></subsection><subsection id="HF11EEC25E2A4458CBE0FA229A0E670F4"><enum>(e)</enum><header>Application</header><text>To seek a grant under this section, an eligible entity shall—</text><paragraph id="H523F3296E08042D4818C1D156DB5FADD"><enum>(1)</enum><text>submit an application at such time, in such manner, and containing such information as the Director may require;</text></paragraph><paragraph id="HCCFB073FC0CB4C04A81290ECF4C14178"><enum>(2)</enum><text display-inline="yes-display-inline">propose a set of activities to address social determinants of health through evidence-based, cross-sector strategies, which activities may include—</text><subparagraph id="H42AECF5B8ECF4E24A5927E01A100CC01"><enum>(A)</enum><text>collecting quantifiable data from health care, social services, and other entities regarding the most significant gaps in health-promoting social, economic, and environmental needs;</text></subparagraph><subparagraph id="H1C19FF1B110C4E8CBEF9B1195C46B46A"><enum>(B)</enum><text>identifying evidence-based approaches to meeting the nonmedical, social needs of populations identified by data collection described in subparagraph (A), such as unstable housing or food insecurity;</text></subparagraph><subparagraph id="HD8D929FA790B449D892F19CC3345037C"><enum>(C)</enum><text display-inline="yes-display-inline">developing scalable methods to meet patients’ social needs identified in clinical settings or other sites;</text></subparagraph><subparagraph id="HA69E8A4912C049E0AB93B612EF3D6F34"><enum>(D)</enum><text display-inline="yes-display-inline">convening entities such as local and State governmental and nongovernmental organizations, health systems, payors, and community-based organizations to review, plan, and implement community-wide interventions and strategies to advance health-promoting social conditions;</text></subparagraph><subparagraph id="HC7EE9C34DD5B417FA6A3638C0061F176"><enum>(E)</enum><text display-inline="yes-display-inline">monitoring and evaluating the impact of activities funded through the grant on the health and well-being of the residents of the target community and on the cost of health care; and </text></subparagraph><subparagraph id="H6F2B52B00BAA4156AF147B9921609B96"><enum>(F)</enum><text display-inline="yes-display-inline">such other activities as may be specified by the Director; </text></subparagraph></paragraph><paragraph commented="no" id="H2178EB1CFD4543348EE13568532666DF"><enum>(3)</enum><text display-inline="yes-display-inline">demonstrate how the eligible entity will collaborate with—</text><subparagraph commented="no" id="HA0B7CB43038B401990CEBE1DB4E83DD3"><enum>(A)</enum><text>health systems;</text></subparagraph><subparagraph commented="no" id="HC5ED488660FB4E62B8E08D11C1ACA2E1"><enum>(B)</enum><text>payors, including, as appropriate, medicaid managed care organizations (as defined in section 1903(m)(1)(A) of the Social Security Act (<external-xref legal-doc="usc" parsable-cite="usc/42/1396b">42 U.S.C. 1396b(m)(1)(A)</external-xref>)), Medicare Advantage plans under part C of title XVIII of such Act (<external-xref legal-doc="usc" parsable-cite="usc/42/1395w-21">42 U.S.C. 1395w–21 et seq.</external-xref>), and health insurance issuers and group health plans (as such terms are defined in section 2791 of the Public Health Service Act (<external-xref legal-doc="usc" parsable-cite="usc/42/300gg-91">42 U.S.C. 300gg–91</external-xref>));</text></subparagraph><subparagraph commented="no" id="HD3D2A7018CAA4B53AF66785F8A73A725"><enum>(C)</enum><text>other relevant stakeholders and initiatives in areas of need, such as the Accountable Health Communities Model of the Centers for Medicare &amp; Medicaid Services, health homes under the Medicaid program under title XIX of the Social Security Act (<external-xref legal-doc="usc" parsable-cite="usc/42/1396">42 U.S.C. 1396 et seq.</external-xref>), community-based organizations, and human services organizations;</text></subparagraph><subparagraph id="H3222EB78EE8649E482615FE0F80659E1"><enum>(D)</enum><text>other non-health care sector organizations, including organizations focusing on transportation, housing, or food access; and</text></subparagraph><subparagraph commented="no" id="H4565FFF6754D44BDA36B80C00E36B58A"><enum>(E)</enum><text>local employers; and</text></subparagraph></paragraph><paragraph id="H0A8E83F9737D4F1EB85561596444101D"><enum>(4)</enum><text display-inline="yes-display-inline">identify key health inequities in the target community and demonstrate how the proposed efforts of the eligible entity would address such inequities.</text></paragraph></subsection><subsection id="H7505EEE5E4164F76870B6B475CF80819"><enum>(f)</enum><header>Monitoring and evaluation</header><text>As a condition of receipt of a grant under this section, a grantee shall agree to submit an annual report to the Director describing the activities carried out through the grant and the outcomes of such activities.</text></subsection><subsection id="H308F63BFE3E145D5A37E598D26BC2594"><enum>(g)</enum><header>Independent national evaluation</header><paragraph id="H46BFBE3804CB440EBF1E857B84737F87"><enum>(1)</enum><header>In general</header><text display-inline="yes-display-inline">Not later than 5 years after the first grants are awarded under this section, the Director shall provide for the commencement of an independent national evaluation of the program under this section.</text></paragraph><paragraph id="H5EC80CDC27044338937695B5BD79F7A8"><enum>(2)</enum><header>Report to Congress</header><text display-inline="yes-display-inline">Not later than 60 days after receiving the results of such independent national evaluation, the Director shall report such results to the Congress.</text></paragraph></subsection></section><section id="HD0705F3727044CCD84F5310C47B4229B"><enum>4.</enum><header>Research and training</header><text display-inline="no-display-inline">The Director, as part of the Program—</text><paragraph id="H265C327D756841B49F038BE51BB237AD"><enum>(1)</enum><text>shall award grants to nonprofit organizations and public or other nonprofit institutions of higher education—</text><subparagraph id="H0A0DE6E5DD254952A5F4C0726FF13C58"><enum>(A)</enum><text display-inline="yes-display-inline">to conduct research on best practices to improve social determinants of health;</text></subparagraph><subparagraph id="H2900504A3D8549089FAA7806E8C586FE"><enum>(B)</enum><text>to provide technical assistance, training, and evaluation assistance to grantees under section 3; and</text></subparagraph><subparagraph id="HD0D6919636C54BD89E8ADD69F2FD6047"><enum>(C)</enum><text>to disseminate best practices to grantees under section 3; and</text></subparagraph></paragraph><paragraph id="HDDB37EA498C84008AEE8B9774D96D9FC"><enum>(2)</enum><text>may require a grantee under paragraph (1) to provide technical assistance and capacity building to entities that are eligible entities under section 3 but not receiving funds through such section.</text></paragraph></section><section id="H8ED3D03A872E492EBFE4656AF84D1883"><enum>5.</enum><header>Funding</header><subsection id="HD8CF92FCCFB745F1A06C47FBC836E2B1"><enum>(a)</enum><header>In general</header><text display-inline="yes-display-inline">There is authorized to be appropriated to carry out this Act, $100,000,000 for each of fiscal years 2025 through 2029.</text></subsection><subsection id="HAD1CC25A9CC54DB4AC75468C5318D628"><enum>(b)</enum><header>Allocation</header><text>Of the amount made available to carry out this Act for a fiscal year, not less than 75 percent shall be used for grants under sections 3 and 4.</text></subsection></section></legis-body></bill>
