which was referred to the <committee-name committee-id="HIF00">Committee on Energy and Commerce</committee-name></action-desc></action><legis-type>A BILL</legis-type><official-title display="yes">To amend the Public Health Service Act to authorize supplemental awards to health centers to establish community transformation hubs.</official-title></form><legis-body id="H1AF7F60E02EF469896F8EF55631BB9C1" style="OLC"><section id="HB23BCB2175354AD1A22E3A59C520B1BA" section-type="section-one"><enum>1.</enum><header>Short title</header><text display-inline="no-display-inline">This Act may be cited as the <quote><short-title>Health Center Community Transformation Hub Act</short-title></quote>.</text></section><section id="H9C0CD78AA8A0455992395348E896AA4D"><enum>2.</enum><header>Supplemental awards to support community transformation hubs</header><text display-inline="no-display-inline">Section 330 of the Public Health Service Act (<external-xref legal-doc="usc" parsable-cite="usc/42/254b">42 U.S.C. 254b</external-xref>) is amended—</text><paragraph id="H6D6DD652C5CA48838C3B4B926A6BE393"><enum>(1)</enum><text>by redesignating subsection (r) as subsection (s); and</text></paragraph><paragraph id="H96F96A966FD64BE98E2EF714248F7B9C"><enum>(2)</enum><text>by inserting after subsection (q) the following:</text><quoted-block style="OLC" id="H295CCCAEDE4C4B519C3BC03988F9C30C" display-inline="no-display-inline"><subsection id="H46006F0464BD45C4AE52C81CDCC761D2"><enum>(r)</enum><header>Supplemental awards To support community transformation</header><paragraph id="H184C1AFD6751470D8DAF0D113B145D70"><enum>(1)</enum><header>In general</header><text display-inline="yes-display-inline">The Secretary may award to a health center receiving a grant under another provision of this section, as a supplement to such grant, funds to implement a community transformation hub to address the social determinants of health faced by patients served by the health center.</text></paragraph><paragraph id="H25BA3CDF1EDD49CE9209D2DBE49DFB0C"><enum>(2)</enum><header>Community transformation hub defined</header><text>In this subsection, the term <term>community transformation hub</term> means a network of community-based organizations capable of addressing primary social determinants of health, including—</text><subparagraph id="H1A21F12E274646759FBD53E9D16213FA"><enum>(A)</enum><text>access to transportation;</text></subparagraph><subparagraph id="H34FA7C66D3804F09B8A00AC4A9478D82"><enum>(B)</enum><text>food security;</text></subparagraph><subparagraph id="HBDBBE889FA13489386FC23EFF6EA9392"><enum>(C)</enum><text>economic security;</text></subparagraph><subparagraph id="H7B55388EC5B645A888529CB46072E197"><enum>(D)</enum><text>education access and quality;</text></subparagraph><subparagraph id="H19BC9C27BA0F4E37AD784EF750C35615"><enum>(E)</enum><text>neighborhood safety;</text></subparagraph><subparagraph id="HECC6126E026F43E3A88CE7C14AEFC506"><enum>(F)</enum><text>systemic racism; and</text></subparagraph><subparagraph id="HEF85BE4FF8E84CDF830000F68F68AB0F"><enum>(G)</enum><text>health of the built environment.</text></subparagraph></paragraph><paragraph id="HC3ED392CFA4349BB98779416B5868CDB"><enum>(3)</enum><header>Consideration</header><text>In making supplemental awards under this subsection, the Secretary shall consider—</text><subparagraph id="H0A7D8FF65897479D8F9EFA1D1ECE388B"><enum>(A)</enum><text>whether the health center involved has conducted an assessment of the social determinants of health facing the health center’s patient population; and</text></subparagraph><subparagraph id="HB797219C8B02426CB5B68DCA45E197F5"><enum>(B)</enum><text display-inline="yes-display-inline">whether the proposed community transformation hub would sufficiently address these determinants.</text></subparagraph></paragraph><paragraph id="HF1F9B4DFB7954A0ABCBBF2300CBBA956"><enum>(4)</enum><header>Partnerships</header><text>The Secretary may allow health centers receiving a supplemental award under this subsection to carry out the activities supported through the award in partnership with community-based organizations through a contract, a subgrant, or a memorandum of understanding.</text></paragraph><paragraph id="HD846A158EC2E48AC851BC241B411FC62"><enum>(5)</enum><header>Technical assistance</header><text display-inline="yes-display-inline">The Secretary may provide technical assistance to award recipients under this subsection with respect to planning, development, and operation of community transformation hubs supported under this subsection.</text></paragraph><paragraph id="H777EF3AD33EE40EF9FFB3AB7BEBC3E49"><enum>(6)</enum><header>Coordination</header><text display-inline="yes-display-inline">Not later than 1 year after the date of enactment of this subsection, the Secretary, acting through the Administrator of the Health Resources and Services Administration and the Administrator of the Centers for Medicare &amp; Medicaid Services, shall issue guidance on sustaining community transformation hubs funded under this section after such funding terminates.</text></paragraph></subsection><after-quoted-block>.</after-quoted-block></quoted-block></paragraph></section></legis-body></bill>
