<official-title>To establish a program to assist States in establishing or enhancing community integration network infrastructure for health and social services.</official-title>

<section id="S1" section-type="section-one"><enum>1.</enum><header>Short title</header><text display-inline="no-display-inline">This Act may be cited as the <quote><short-title>Leveraging Integrated Networks in Communities to Address Social Needs Act of 2021</short-title></quote> or the <quote><short-title>LINC to Address Social Needs Act of 2021</short-title></quote>.</text></section> <section id="id7A30227C5B55400895CCB6EBE426668F"><enum>2.</enum><header>Program to support establishment or enhancement of State community integration network infrastructure</header> <subsection id="id0CD6B460BE80449A947A0069440979DD"><enum>(a)</enum><header>Grant program</header><text>The Secretary shall award grants on a competitive basis to States to support such States, through public-private partnerships, to establish new or enhance existing community integration network infrastructure through authorized activities under subsection (b).</text></subsection>
<subsection id="idA1C1073DA11B42D0A717F7BCF6D27A35"><enum>(b)</enum><header>Authorized activities</header><text>A State shall use a grant under this section to carry out activities and services to establish new or enhance existing community integration network infrastructure, on a statewide basis through direct network operations or collaborations among multiple associated entities, which may include such entities that operate regionally. Such activities and services shall include—</text> <paragraph id="id9db7e897ab934e539f0f7c133664c739"><enum>(1)</enum><text>establishing a new or enhancing an existing technology platform that—</text>
<subparagraph id="id2ad37bc1b6264907aab16b6f931e01b0"><enum>(A)</enum><text>enables the coordination of public and private providers and payors of services for individuals in the State, including services such as—</text> <clause id="id1B174ADEB44B4B66BDC471331970BDA7"><enum>(i)</enum><text>nutritional assistance; </text></clause>
<clause id="id951D911235634951BB3CD1A4C8F69635"><enum>(ii)</enum><text>housing;</text></clause> <clause id="id8ACF61AD49B04F958ED16B21BF81DD2C"><enum>(iii)</enum><text>health care, including preventive health intervention, chronic disease management, and behavioral health care; </text></clause>
<clause id="id1D545B6E6C034D73BD48914D8AE29C41"><enum>(iv)</enum><text>transportation; </text></clause> <clause id="id47CD4991486648E59AD8E0C21988DC64"><enum>(v)</enum><text>job training; </text></clause>
<clause id="id2E587B12A75A408D826206CBB1C15EE5"><enum>(vi)</enum><text>child development or care;</text></clause> <clause id="id65E549F8D87B46CC92D0FB9AF4A4A2C0"><enum>(vii)</enum><text>caregiving and respite care;</text></clause>
<clause id="id1EAC84CEF21A4129AEC5B09C63A79162"><enum>(viii)</enum><text>disability assistance; and</text></clause> <clause id="idA583D937460A495A88CE6F0FC07BE953"><enum>(ix)</enum><text>other services, as determined by the State;</text></clause></subparagraph>
<subparagraph id="idc35d7224c53b476faf2bc2f947f873cb"><enum>(B)</enum><text>prioritizes connectivity with, incorporation of, and partnership with any appropriate existing technology platforms developed by public or private organizations in the State for the purposes described in subparagraph (A); and</text></subparagraph> <subparagraph id="ide46e8b3bba9b4411b5d5800064a6e247"><enum>(C)</enum><text>ensures that—</text>
<clause id="id33299c452e194b04ba5e5f3234022d96"><enum>(i)</enum><text>reasonable measures are taken to promote connectivity among associated entities; and</text></clause> <clause id="id82425ed3945547bb9d0a4db735fd3414"><enum>(ii)</enum><text>appropriate privacy, security, protections are in place, in accordance with applicable Federal and State privacy laws;</text></clause></subparagraph></paragraph>
<paragraph id="idab92d8ed6c214cbd93f459347409dcfb"><enum>(2)</enum><text>connecting associated entities for purposes of communication, service coordination and consumer assistance, referral and capacity management, outcome tracking, and related services;</text></paragraph> <paragraph id="idb1a4c2ba86854bcab623c2ef86f4ed1a"><enum>(3)</enum><text>providing technical assistance and supporting associated entities in connecting and participating in the community integration network infrastructure;</text></paragraph>
<paragraph id="idd7d95a904a4f4284a4b8696d9ec2a3dc"><enum>(4)</enum><text>planning for and implementing actions designed to create sustainable funding models to support long-term access to community integration network infrastructure; </text></paragraph> <paragraph id="id4F82784AAAC9455C8E630A62E680A3E4"><enum>(5)</enum><text>designing and implementing a financial structure to make the community integration network infrastructure financially self-sustaining not later than 3 years after receiving funds under this section; and</text></paragraph>
<paragraph id="idD8C23CA93FB745C6BF6201E8D6577D8E"><enum>(6)</enum><text>evaluating the use of any funds provided under this section.</text></paragraph></subsection> <subsection id="idfecce999827a4759a4da5a0ebe72ec94"><enum>(c)</enum><header>Award of grants</header> <paragraph id="idDEFC5E6DA1F54ADFB8B13576E1676226"><enum>(1)</enum><header>In general</header><text>A grant under this Act shall be awarded under such terms and conditions as the Secretary shall prescribe, including the guidelines established under paragraph (3). </text></paragraph>
<paragraph id="id4FA562490CC9484B88E4EB7997B68231" commented="no" display-inline="no-display-inline"><enum>(2)</enum><header>Minimizing administrative burden</header><text>The Secretary shall seek to minimize the administrative burden of such terms and conditions and ensure programmatic flexibility for unique State needs.</text></paragraph> <paragraph id="id3e3792b4d9824a018da63cd71a4e9ebb"><enum>(3)</enum><header>Guidelines</header><text>The Secretary shall—</text>
<subparagraph id="id5B720E5370C34358B5446FCE7A9366B0"><enum>(A)</enum><text>consult relevant stakeholders regarding basic functionalities, technical capacities, and data standards needed for community integration network infrastructure; and </text></subparagraph> <subparagraph id="id6107200B6DB1470594D1D4712455F517"><enum>(B)</enum><text>based on such consultations, establish guidelines for awarding grants under this section, including application requirements. </text></subparagraph></paragraph></subsection>
<subsection id="ID54BC29C91CA040E18785FFBBA25C8749"><enum>(d)</enum><header>Application</header><text>A State desiring a grant under this section shall—</text> <paragraph id="idA971EB6666B543EBB7231E0FFEF3A755"><enum>(1)</enum><text>enter into a public-private partnership with one or more—</text>
<subparagraph id="id4AA31959A2864310B3210062BFE19754"><enum>(A)</enum><text>private, nonprofit, or philanthropic organizations; or</text></subparagraph> <subparagraph id="id2C0DFCEB42D149D7BBF5AEC145CAD19B" commented="no" display-inline="no-display-inline"><enum>(B)</enum><text>Indian Tribes, Tribal organizations, or urban Indian organizations within the State; and</text></subparagraph></paragraph>
<paragraph id="idBE37CFBC1FE74AC1AC8F592FBC42D156"><enum>(2)</enum><text>submit to the Secretary an application at such time, in such manner, and containing or accompanied by such information as the Secretary may require, including—</text> <subparagraph id="IDCFF273534C344236AE2778ABE39A6053"><enum>(A)</enum><text>a description of the agency or department in the State government that will coordinate with and oversee the partnership established under paragraph (1);</text></subparagraph>
<subparagraph id="id336058ACB59946BCA2278E5C8DBC8A1C"><enum>(B)</enum><text> a plan for the establishment or enhancement of a community integration network infrastructure including—</text> <clause id="id7121DC27761E45ACA80F5C1DA9967529"><enum>(i)</enum><text>the proposed transparent and competitive process for selecting any new operational components of the community integration network infrastructure; </text></clause>
<clause id="id708B0DDD10984951B7895B56153DD2FB"><enum>(ii)</enum><text>the planned governance structure (including representation of different types of associated entities) within the community integration network infrastructure; </text></clause> <clause id="idD6088431893E4B52B6F4A6D73E6641CA"><enum>(iii)</enum><text>proposed associated entities and services to be included in the community integration network infrastructure; and</text></clause>
<clause id="id4B480E11AD42430C89E68D3028924269"><enum>(iv)</enum><text>a plan for accessing and linking relevant data to create community integration network infrastructure, including a description of intended sources of data;</text></clause></subparagraph> <subparagraph id="idF997BB9B2D3B4764979293E5ACA5DA5B"><enum>(C)</enum><text>assurances that the funds awarded under this section will be used solely carry out authorized activities as described in subsection (b) and other related activities;</text></subparagraph>
<subparagraph id="idfcc4d6f2928344d5a06956d516a294a7"><enum>(D)</enum><text>potential options, including public-private partnerships in addition to the partnership described in paragraph (1), for making the community integration network infrastructure financially self-sustaining not later than 3 years after receiving funds under this section; and</text></subparagraph> <subparagraph id="id7f1e6612c59a48d5a618b9d9c78bdc30"><enum>(E)</enum><text>a description of the objectives and outcome goals of developing the community integration network infrastructure, including—</text>
<clause id="id5EB928312CC949ABA6D1E0621AEC2005"><enum>(i)</enum><text>one or more health outcomes; </text></clause> <clause id="id166F44AFBF8F4B949E0C698CA04F360C"><enum>(ii)</enum><text>one or more other important social outcomes; </text></clause>
<clause id="id90CBA0E9AB2B4978904D8CCFCEADEF9D"><enum>(iii)</enum><text>improved access to health care or social services; and </text></clause> <clause id="id347F2691061640E490EA2BE8C674C375"><enum>(iv)</enum><text>how progress toward the outcomes described in subparagraphs (A), (B), and (C) will be measured through internal performance metrics.</text></clause></subparagraph></paragraph></subsection>
<subsection id="id1440BB27A0004CE8AF37AB66C5BA4949"><enum>(e)</enum><header>Separate tribal infrastructure</header><text>Nothing in this section shall preclude Indian Tribes, Tribal organizations, or urban Indian organizations from establishing a community integration network infrastructure that is separate from any other public-private partnership receiving funding under this section. </text></subsection> <subsection id="id30415d9567b94530874275c952a70d44"><enum>(f)</enum><header>Authorization of appropriations</header> <paragraph id="id27b5c8cea74e487fa966d4bb4f8b34f5"><enum>(1)</enum><header>In general</header><text>There is authorized to be appropriated to carry out this section $150,000,000 for fiscal year 2022.</text></paragraph>
<paragraph id="id1B48026116644F78A7301E641AE14757"><enum>(2)</enum><header>Administration</header><text>Of the amounts appropriated pursuant to paragraph (1), up to $5,000,000 may be used for administrative expenses.</text></paragraph> <paragraph id="id68457ef9ac2a49fd84fcc5f740d41f64"><enum>(3)</enum><header>Duration of availability</header><text>Amounts appropriated pursuant to paragraph (1) shall remain available until the date that is 5 years after the date of enactment of this Act.</text></paragraph></subsection>
<subsection id="id3624F39A35AE4D63BC47734592238852"><enum>(g)</enum><header>Definitions</header><text>In this section:</text> <paragraph id="idDB59DC69A61C45FBBD76627ACA9C2ECB"><enum>(1)</enum><header>Associated entities</header><text>The term <term>associated entities</term> means any—</text>
<subparagraph id="idaa8cfe5f417f4574b7bbb783df087ffc"><enum>(A)</enum><text>community-based organization that accepts referrals from health care organizations and that provides services such as—</text> <clause id="id631d1829203841dfb95687bdc05a8fa1"><enum>(i)</enum><text>nutritional assistance; </text></clause>
<clause id="idded8cdad2f3440cfa3f80d8a28e4f0f0"><enum>(ii)</enum><text>housing;</text></clause> <clause id="id6e06357b14544f839a2b8f08a3a49242"><enum>(iii)</enum><text>health care, including preventive health intervention, chronic disease management, and behavioral health care; </text></clause>
<clause id="id83cfc9211a5c4e07b0f9737e397e3537"><enum>(iv)</enum><text>transportation; </text></clause> <clause id="id65bf3ceafa594255bfc5acc6ebf865f8"><enum>(v)</enum><text>job training; </text></clause>
<clause id="id1e890faddde14901b138475d5321e33a"><enum>(vi)</enum><text>child development or care;</text></clause> <clause id="id5a4f9df2c707464aa5f4f95d749b723d"><enum>(vii)</enum><text>caregiving and respite care; and</text></clause>
<clause id="idf6b335a6ef1f4eacb01ab2d3158aef42"><enum>(viii)</enum><text>disability assistance;</text></clause></subparagraph> <subparagraph id="id2efc227cffc34782b3e93c70c7245be8"><enum>(B)</enum><text>public, or nonprofit or for-profit, private health care provider organization;</text></subparagraph>
<subparagraph id="idd4304e873baa46acb5b03deb0f0f9cfc"><enum>(C)</enum><text>public or private funded payor of health care services, including home- or community-based services;</text></subparagraph> <subparagraph id="id2d43980ececd4ee6b27aa8380afb34c9"><enum>(D)</enum><text>State, local, territorial, or Tribal health and social services agency;</text></subparagraph>
<subparagraph id="id3b3a6c43448a46c0872ae15b1d3c4373"><enum>(E)</enum><text>State public housing authority or housing finance agency;</text></subparagraph> <subparagraph id="id7A3FE57EB8704EAA8F27D4FD6AE64EF8"><enum>(F)</enum><text>public health information exchange or public health information network, as defined by the Secretary; or</text></subparagraph>
<subparagraph id="id4f2fa39bab6f480bafc09b1bae11aeff"><enum>(G)</enum><text>other similar entity, as designated by the State.</text></subparagraph></paragraph> <paragraph id="idB84B9AAC25084F88A14774FAC474AA69"><enum>(2)</enum><header>Community integration network infrastructure</header><text>The term <term>community integration network infrastructure</term> means infrastructure, existing on statewide basis with direct network operations or through collaborations among multiple associated entities, used to enable the coordination, alignment, and connection, of associated entities in a State, including such entities that operate regionally, for purposes of communication, service coordination, and referral management of services, with respect to services such as—</text>
<subparagraph id="id40435c2178574c6f90456191a9eefd7d"><enum>(A)</enum><text>nutritional assistance; </text></subparagraph> <subparagraph id="idf184c583ed38457d915196c8d3c2a683"><enum>(B)</enum><text>housing;</text></subparagraph>
<subparagraph id="id187f0ae0a50a4810b0b50eb060213701"><enum>(C)</enum><text>health care, including preventive health intervention, chronic disease management, and behavioral health care; </text></subparagraph> <subparagraph id="id3b387cd78f8d405bacaa839eb3663cc3"><enum>(D)</enum><text>transportation; </text></subparagraph>
<subparagraph id="id18f792b267b34ea7995afd46bf922091"><enum>(E)</enum><text>job training; </text></subparagraph> <subparagraph id="id8e2a8aae5eab47b5b8d67ab91c66182d"><enum>(F)</enum><text>child development or care;</text></subparagraph>
<subparagraph id="idaa130f28d9eb4619bb8967d11ce79e80"><enum>(G)</enum><text>caregiving and respite care;</text></subparagraph> <subparagraph id="id39eb3bbae4a8457ba244c845cd2931be"><enum>(H)</enum><text>disability assistance; and</text></subparagraph>
<subparagraph id="id7AC25A865E7E44E9A447FF0DF04DF7CA"><enum>(I)</enum><text>other similar services, as designated by the State.</text></subparagraph></paragraph> <paragraph id="idD8B1D17FFE224A6BA5F9C7674134D920" commented="no"><enum>(3)</enum><header>Indian tribe and tribal organization</header><text>The terms <term>Indian Tribe</term> and <term>Tribal organization</term> have the meanings given to the terms ‘Indian tribe’ and ‘tribal organization’ in section 4 of the Indian Self-Determination and Education Assistance Act (<external-xref legal-doc="usc" parsable-cite="usc/25/5304">25 U.S.C. 5304</external-xref>).</text></paragraph>
<paragraph id="id15ab2960035d4bb2a08c24477ecf1d2f"><enum>(4)</enum><header>Secretary</header><text>The term <term>Secretary</term> refers to the Secretary of Health and Human Services. </text></paragraph> <paragraph id="idD35300697C3B406DAEDA2641B1832981"><enum>(5)</enum><header>State</header><text>The term <term>State</term> means a State, territory, or the District of Columbia.</text></paragraph>
<paragraph id="id8793CA297C394E6FB80A2DF1CFB97311" commented="no" display-inline="no-display-inline"><enum>(6)</enum><header>Urban indian organization</header><text>The term <term>urban Indian organization</term> has the meaning given to the term in section 4 of the Indian Health Care Improvement Act (<external-xref legal-doc="usc" parsable-cite="usc/25/1603">25 U.S.C. 1603</external-xref>).</text></paragraph></subsection></section> <section id="id6004AEFD660C4A02AB888D069CA8B8BB"><enum>3.</enum><header>Evaluation, report, and recommendations</header> <subsection id="idB6278CA915B34191A223403693E33A18"><enum>(a)</enum><header>Evaluation</header><text display-inline="yes-display-inline">The Comptroller General of the United States shall conduct an evaluation that—</text>
<paragraph id="id3DFE2E668B344710BAB9BD929079CE86"><enum>(1)</enum><text>measures the overall impact of the community integration network infrastructure established or enhanced using funds received under section 2, with respect to—</text> <subparagraph id="id844989CE03594ED880E689FE8F0A4FCB"><enum>(A)</enum><text>changes in individual and population health outcomes;</text></subparagraph>
<subparagraph id="idFEF0771AA1AC49829BE12F43BE7AE717"><enum>(B)</enum><text>changes in access to health care or social services;</text></subparagraph> <subparagraph id="ide3df524e132c42afaf841fd52e2dd398"><enum>(C)</enum><text>the degree of data sharing using the community integration network infrastructure established or enhanced using funds received under section 2;</text></subparagraph>
<subparagraph id="id4c2d14d6f8bd48eb8b7e307260bcda5c"><enum>(D)</enum><text>the effectiveness of service coordination; </text></subparagraph> <subparagraph id="ida62d69812909434e888a1cf4cdb78cbe"><enum>(E)</enum><text>the cost-effectiveness of the provision of services;</text></subparagraph>
<subparagraph id="id1af4d2d6fd524adfbdc31d5c2f355aba" commented="no" display-inline="no-display-inline"><enum>(F)</enum><text>any results or anticipated results on overall health and social services spending;</text></subparagraph> <subparagraph commented="no" display-inline="no-display-inline" id="idCC4F6514E2B44D15B2ABFA0766D9CC7D"><enum>(G)</enum><text>patient and consumer satisfaction with service coordination process and services received; and </text></subparagraph>
<subparagraph commented="no" display-inline="no-display-inline" id="idF4D5FE4F643642B79BE03FB8B66D0189"><enum>(H)</enum><text>any other relevant factors; and</text></subparagraph></paragraph> <paragraph display-inline="no-display-inline" commented="no" id="id53363093C4D64068BF18B0DE7CAA0141"><enum>(2)</enum><text>describes how the funds received under section 2 were used.</text></paragraph></subsection>
<subsection id="idDE74E8E5DBD94D089932566B7E7CD8F6"><enum>(b)</enum><header>Report and recommendations</header><text>Not later than 4 years after the date the first grant under this Act is awarded, the Comptroller General of the United States shall—</text> <paragraph id="id796EC10582FA4DD29E6EB7619789283A"><enum>(1)</enum> <subparagraph commented="no" display-inline="yes-display-inline" id="id6F25D63FDAC247BEAA1900856D4C74A5"><enum>(A)</enum><text>submit a report on the evaluation conducted under subsection (a) to Congress; and</text></subparagraph>
<subparagraph id="id2B1D05F5FC66456ABE79525F52FB7487" indent="up1"><enum>(B)</enum><text>make such report publicly available; and</text></subparagraph></paragraph> <paragraph id="idB3D3BFC255604F2FAB3A92A4FF19BBC8"><enum>(2)</enum><text>based on the evaluation conducted under subsection (a), make recommendations to States and Indian Tribes, Tribal organizations, or urban Indian organizations, on how to improve and sustain community integration network infrastructure established or enhanced using funds received under section 2.</text></paragraph></subsection></section>
