			 Clinical Care Commission. </official-title></form>

 <section id="HE0FFF9FA71944025924EE3CC17B2FF91" section-type="section-one"><enum>1.</enum><header>Short title</header><text display-inline="no-display-inline">This Act may be cited as the <quote><short-title>National Clinical Care Commission Act</short-title></quote>.</text> </section><section id="H48CB2846F1D2436BB4554E8070A70AE8"><enum>2.</enum><header>National clinical care commission</header> <subsection id="HACB38EBDDF094BD69BC457037961EA46"><enum>(a)</enum><header>Establishment</header><text>There is hereby established, within the Department of Health and Human Services, a National Clinical Care Commission (in this section referred to as the <quote>Commission</quote>) to evaluate and make recommendations regarding improvements to the coordination and leveraging of programs within the Department and other Federal agencies related to awareness and clinical care for at least one, but not more than two, complex metabolic or autoimmune diseases resulting from issues related to insulin that represent a significant disease burden in the United States, which may include complications due to such diseases.</text>
			</subsection><subsection id="H08FAA6D308184799A0DB05FFDEA1B40A"><enum>(b)</enum><header>Membership</header>
 <paragraph id="HE6BBD1E8746A43E4BDAFCE52B883A0A7"><enum>(1)</enum><header>In general</header><text>The Commission shall be composed of the following voting members:</text> <subparagraph id="HAF377ECEF89C4EF09F1914D9B95695AC"><enum>(A)</enum><text>The heads of the following Federal agencies and departments, or their designees:</text>
 <clause id="HEC0A44D7A986472D92961E8C4BD8B79F"><enum>(i)</enum><text>The Centers for Medicare &amp; Medicaid Services.</text> </clause><clause id="H26D18C1A0F6241F68C201A91DEC8B05A"><enum>(ii)</enum><text>The Agency for Healthcare Research and Quality.</text>
 </clause><clause id="H7E2E971693574B4A8ABDDE668D58B0FD"><enum>(iii)</enum><text>The Centers for Disease Control and Prevention.</text> </clause><clause id="H78062835AEB6459EA6B8BE1A443068DF"><enum>(iv)</enum><text>The Indian Health Service.</text>
 </clause><clause id="H076C69D2A89444C38FAD5AD889CAC994"><enum>(v)</enum><text>The Department of Veterans Affairs.</text> </clause><clause id="HAD8E9A2F11CE4CE5839E4B9AE69C851A"><enum>(vi)</enum><text>The National Institutes of Health.</text>
 </clause><clause id="HA88F2C1B1E0647EB8E16B4573688D9F0"><enum>(vii)</enum><text>The Food and Drug Administration.</text> </clause><clause id="H260BD0B065A34012B4B0C8C1F9482935"><enum>(viii)</enum><text>The Health Resources and Services Administration.</text>
 </clause><clause id="HA8B5A0D8575D4CD3AEDE042B0BC0DFBE"><enum>(ix)</enum><text>The Department of Defense.</text> </clause><clause id="id8CAC23F1EDA844618F226129309C07B7"><enum>(x)</enum><text>The Department of Agriculture.</text>
 </clause><clause id="id178F3E4BF7F9481A865C5AB47140C93E"><enum>(xi)</enum><text>The Office of Minority Health.</text> </clause></subparagraph><subparagraph id="H1008FFD6A8A9468C8EECF2BF8A5AD493"><enum>(B)</enum><text>Twelve additional voting members appointed under paragraph (2).</text>
 </subparagraph></paragraph><paragraph id="HCFFE1BE7A0714AEEB81A640AC6CE4515"><enum>(2)</enum><header>Additional members</header><text>The Commission shall include additional voting members, as may be appointed by the Secretary, with expertise in the prevention, care, and epidemiology of any of the diseases and complications described in subsection (a), including one or more such members from each of the following categories:</text>
 <subparagraph id="H9562E7675F804E0780F29D1043D852AC"><enum>(A)</enum><text>Physician specialties, including clinical endocrinologists, that play a role in the prevention or treatment of diseases and complications described in subsection (a).</text>
 </subparagraph><subparagraph id="HAE1AEE325818420FA99A8783470E3A99"><enum>(B)</enum><text>Primary care physicians.</text> </subparagraph><subparagraph id="H45311C6E298344A88D88462C25C88A3C"><enum>(C)</enum><text>Non-physician health care professionals.</text>
 </subparagraph><subparagraph id="H47F81F49FEE143D4B66A2F840AAD3519"><enum>(D)</enum><text>Patient advocates.</text> </subparagraph><subparagraph id="HD7FFCA0B926C405FA5CF6B53B17FC416"><enum>(E)</enum><text>National experts, including public health experts, in the duties listed under subsection (c).</text>
 </subparagraph><subparagraph id="H7E71FF2AFE5B4D298CB549623222630F"><enum>(F)</enum><text>Health care providers furnishing services to a patient population that consists of a high percentage (as specified by the Secretary) of individuals who are enrolled in a State plan under title XIX of the Social Security Act or who are not covered under a health plan or health insurance coverage.</text>
 </subparagraph></paragraph><paragraph id="HD63B6B96119F4FAFBCA84B6E61AD4A62"><enum>(3)</enum><header>Chairperson</header><text>The members of the Commission shall select a chairperson from the members appointed under paragraph (2).</text>
 </paragraph><paragraph id="H30CE6BCAB34A450D9F440A3392BE10B9"><enum>(4)</enum><header>Meetings</header><text>The Commission shall meet at least twice, and not more than four times, a year.</text> </paragraph><paragraph id="H386B3CE951EC4C048270809CE5139778"><enum>(5)</enum><header>Vacancies</header><text>A vacancy on the Commission shall be filled in the same manner as the original appointments.</text>
 </paragraph></subsection><subsection id="H441707B068D0437CA32C668C1D0AF182"><enum>(c)</enum><header>Duties</header><text>The Commission shall evaluate and make recommendations, as appropriate, to the Secretary of Health and Human Services and Congress regarding—</text>
 <paragraph id="H2BD31ACD5D5C4065997502BD0B55D8B4"><enum>(1)</enum><text>Federal programs of the Department of Health and Human Services that focus on preventing and reducing the incidence of the diseases and complications described in subsection (a);</text>
 </paragraph><paragraph id="H5A0EE9AD78B04D369227A850EB8D6B90"><enum>(2)</enum><text>current activities and gaps in Federal efforts to support clinicians in providing integrated, high-quality care to individuals with the diseases and complications described in subsection (a);</text>
 </paragraph><paragraph id="H9583850419C44BDB81B0458E2930FCE8"><enum>(3)</enum><text>the improvement in, and improved coordination of, Federal education and awareness activities related to the prevention and treatment of the diseases and complications described in subsection (a), which may include the utilization of new and existing technologies; and</text>
 </paragraph><paragraph id="H1EB0236353394A0F9723F58C20FC8B0F"><enum>(4)</enum><text>methods for outreach and dissemination of education and awareness materials that—</text> <subparagraph id="H14A9E404AE384F9CA729FB1A9C13BDCC"><enum>(A)</enum><text>address the diseases and complications described in subsection (a);</text>
 </subparagraph><subparagraph id="HED32ED62AE7547E1A9300BF016BCC9F5"><enum>(B)</enum><text>are funded by the Federal Government; and</text> </subparagraph><subparagraph id="H44888A21B40C4634BE437F330D8E121E"><enum>(C)</enum><text>are intended for health care professionals and the public.</text>
 </subparagraph></paragraph></subsection><subsection id="H67CFB16A5CDF40E7BE0D224D9CB50C9C"><enum>(d)</enum><header>Operating plan</header><text>Not later than 90 days after its first meeting, the Commission shall submit to the Secretary of Health and Human Services and the Congress an operating plan for carrying out the activities of the Commission as described in subsection (c). Such operating plan may include—</text>
 <paragraph id="H77DA9DD6C799481E96E1887C2B819F40"><enum>(1)</enum><text>a list of specific activities that the Commission plans to conduct for purposes of carrying out the duties described in each of the paragraphs in subsection (c);</text>
 </paragraph><paragraph id="H6F38BD565B7042CF9F131A540F142FF8"><enum>(2)</enum><text>a plan for completing the activities;</text> </paragraph><paragraph id="H445B6BB771684FAB97042A743ABF185B"><enum>(3)</enum><text>a list of members of the Commission and other individuals who are not members of the Commission who will need to be involved to conduct such activities;</text>
 </paragraph><paragraph id="H4923EFF376BF4140B0E4B6CD9A9D6BCA"><enum>(4)</enum><text>an explanation of Federal agency involvement and coordination needed to conduct such activities;</text> </paragraph><paragraph id="H335A19A4CB7A493C8DD75310C7B0D0E2"><enum>(5)</enum><text>a budget for conducting such activities; and</text>
 </paragraph><paragraph id="HECFCA22D983B44D298E4044E8F879CF0"><enum>(6)</enum><text>other information that the Commission deems appropriate.</text> </paragraph></subsection><subsection id="H7DFF95B24F144D5A98308E0EBC219ED5"><enum>(e)</enum><header>Final report</header><text>By not later than 3 years after the date of the Commission’s first meeting, the Commission shall submit to the Secretary of Health and Human Services and the Congress a final report containing all of the findings and recommendations required by this section.</text>
 </subsection><subsection commented="no" display-inline="no-display-inline" id="HBB1D44041F7E42E28CFE1294F594E888"><enum>(f)</enum><header>Sunset</header><text>The Commission shall terminate 60 days after submitting its final report, but not later than the end of fiscal year 2021.</text></subsection></section></legis-body></bill>
