<section id="d226354e90" identifier="/us/pl/116/212/s1" style="-uslm-lc:I658146"><num class="bold" value="1">SECTION 1. </num><heading>SHORT TITLE.</heading><content style="-uslm-lc:I658120">  This Act may be cited as the “<shortTitle role="act">Improving Safety and Security for Veterans Act of 2019</shortTitle>”.</content></section>
<section id="d226354e100" identifier="/us/pl/116/212/s2" style="-uslm-lc:I658141"><num class="fontsize12" value="2">SEC. 2. </num><heading>DEPARTMENT OF VETERANS AFFAIRS REPORTS ON PATIENT SAFETY AND QUALITY OF CARE.</heading><subsection class="firstIndent0 fontsize10" id="y14d61107-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a" style="-uslm-lc:I658120"><num class="fontsize10" style="-uslm-lc:emspace2" value="a">(a) </num><heading class="fontsize10"><inline class="smallCaps">Report on Patient Safety and Quality of Care</inline>.—</heading><paragraph class="fontsize10" id="y14d61108-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/1" style="-uslm-lc:I658122"><num class="fontsize10" style="-uslm-lc:emspace2" value="1">(1) </num><heading class="fontsize10"><inline class="smallCaps">In general</inline>.—</heading><content>Not later than 30 days after the date of the enactment of this Act, the Secretary of Veterans Affairs shall submit to the Committee on Veterans’ Affairs of the Senate and the Committee on Veterans’ Affairs of the House of Representatives a report regarding the policies and procedures of the Department relating to patient safety and quality of care and the steps that the Department has taken to make improvements in patient safety and quality of care at medical centers of the Department.</content></paragraph>
<paragraph class="fontsize10" id="y14d61109-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2" style="-uslm-lc:I658122"><num class="fontsize10" style="-uslm-lc:emspace2" value="2">(2) </num><heading class="fontsize10"><inline class="smallCaps">Elements</inline>.—</heading><chapeau>The report required by paragraph (1) shall include the following:</chapeau><subparagraph class="fontsize10" id="y14d6110a-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="A">(A) </num><chapeau>A description of the policies and procedures of the Department and improvements made by the Department with respect to the following:</chapeau><clause class="fontsize10" id="y14d6110b-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/i" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="i">(i) </num><content>How often the Department reviews or inspects patient safety at medical centers of the Department.</content></clause>
<clause class="fontsize10" id="y14d6110c-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/ii" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="ii">(ii) </num><content>What triggers the aggregated review process at medical centers of the Department.</content></clause>
<clause class="fontsize10" id="y14d6110d-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/iii" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="iii">(iii) </num><chapeau>What controls the Department has in place for controlled and other high-risk substances, including the following:</chapeau><subclause class="fontsize10" id="y14d6110e-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/iii/I" style="-uslm-lc:I658128"><num class="fontsize10" style="-uslm-lc:emspace2" value="I">(I) </num><content>Access to such substances by staff.</content></subclause>
<subclause class="fontsize10" id="y14d6110f-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/iii/II" style="-uslm-lc:I658128"><num class="fontsize10" style="-uslm-lc:emspace2" value="II">(II) </num><content>What medications are dispensed via automation.</content></subclause>
<subclause class="fontsize10" id="y14d61110-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/iii/III" style="-uslm-lc:I658128"><num class="fontsize10" style="-uslm-lc:emspace2" value="III">(III) </num><content>What systems are in place to ensure proper matching of the correct medication to the correct patient.</content></subclause>
<subclause class="fontsize10" id="y14d61111-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/iii/IV" style="-uslm-lc:I658128"><num class="fontsize10" style="-uslm-lc:emspace2" value="IV">(IV) </num><content>Controls of items such as medication carts and pill bottles and vials.<page identifier="/us/stat/134/1020">134 STAT. 1020</page></content></subclause>
<subclause class="fontsize10" id="y14d61112-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/iii/V" style="-uslm-lc:I658128"><num class="fontsize10" style="-uslm-lc:emspace2" value="V">(V) </num><content>Monitoring of the dispensing of medication within medical centers of the Department, including monitoring of unauthorized dispensing.</content></subclause>
</clause>
<clause class="fontsize10" id="y14d61113-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/iv" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="iv">(iv) </num><content>How the Department monitors contact between patients and employees of the Department, including how employees are monitored and tracked at medical centers of the Department when entering and exiting the room of a patient.</content></clause>
<clause class="fontsize10" id="y14d61114-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/v" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="v">(v) </num><content>How comprehensively the Department uses video monitoring systems in medical centers of the Department to enhance patient safety, security, and quality of care.</content></clause>
<clause class="fontsize10" id="y14d61115-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/vi" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="vi">(vi) </num><content>How the Department tracks and reports deaths at medical centers of the Department at the local level, Veterans Integrated Service Network level, and national level.</content></clause>
<clause class="fontsize10" id="y14d61116-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/vii" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="vii">(vii) </num><sidenote><p class="leftAlign firstIndent0 fontsize8" id="x14d61117-e826-11f0-bc57-ad3ac4b1618c" style="-uslm-lc:I658180">Procedures.</p></sidenote><chapeau>The procedures of the Department to alert local, regional, and Department-wide leadership when there is a statistically abnormal number of deaths at a medical center of the Department, including—</chapeau><subclause class="fontsize10" id="y14d61118-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/vii/I" style="-uslm-lc:I658128"><num class="fontsize10" style="-uslm-lc:emspace2" value="I">(I) </num><content>the manner and frequency in which such alerts are made; and</content></subclause>
<subclause class="fontsize10" id="y14d61119-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/vii/II" style="-uslm-lc:I658128"><num class="fontsize10" style="-uslm-lc:emspace2" value="II">(II) </num><content>what is included in such an alert, such as the nature of death and where within the medical center the death occurred.</content></subclause>
</clause>
<clause class="fontsize10" id="y14d6111a-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/viii" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="viii">(viii) </num><sidenote><p class="leftAlign firstIndent0 fontsize8" id="x14d6111b-e826-11f0-bc57-ad3ac4b1618c" style="-uslm-lc:I658180">Analyses.</p></sidenote><chapeau>The use of root cause analyses with respect to patient deaths in medical centers of the Department, including—</chapeau><subclause class="fontsize10" id="y14d6111c-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/viii/I" style="-uslm-lc:I658128"><num class="fontsize10" style="-uslm-lc:emspace2" value="I">(I) </num><content>what threshold triggers a root cause analysis for a patient death;</content></subclause>
<subclause class="fontsize10" id="y14d6111d-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/viii/II" style="-uslm-lc:I658128"><num class="fontsize10" style="-uslm-lc:emspace2" value="II">(II) </num><content>who conducts the root cause analysis; and</content></subclause>
<subclause class="fontsize10" id="y14d6111e-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/viii/III" style="-uslm-lc:I658128"><num class="fontsize10" style="-uslm-lc:emspace2" value="III">(III) </num><content>how root cause analyses determine whether a patient death is suspicious or not.</content></subclause>
</clause>
<clause class="fontsize10" id="y14d6111f-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/ix" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="ix">(ix) </num><content>What triggers a patient safety alert, including how many suspicious deaths cause a patient safety alert to be triggered.</content></clause>
<clause class="fontsize10" id="y14d61120-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/x" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="x">(x) </num><chapeau>The situations in which an autopsy report is ordered for deaths at hospitals of the Department, including an identification of—</chapeau><subclause class="fontsize10" id="y14d61121-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/x/I" style="-uslm-lc:I658128"><num class="fontsize10" style="-uslm-lc:emspace2" value="I">(I) </num><content>when the medical examiner is called to review a patient death; and</content></subclause>
<subclause class="fontsize10" id="y14d61122-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/x/II" style="-uslm-lc:I658128"><num class="fontsize10" style="-uslm-lc:emspace2" value="II">(II) </num><content>the official or officials that decide such a review is necessary.</content></subclause>
</clause>
<clause class="fontsize10" id="y14d61123-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/xi" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="xi">(xi) </num><content>The method for family members of a patient who died at a medical center of the Department to request an investigation into that death.</content></clause>
<clause class="fontsize10" id="y14d61124-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/xii" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="xii">(xii) </num><content>The opportunities that exist for family members of a patient who died at a medical center of the Department to request an autopsy for that death.</content></clause>
<clause class="fontsize10" id="y14d61125-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/xiii" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="xiii">(xiii) </num><content>The methods in place for employees of the Department to report suspicious deaths at medical centers of the Department.</content></clause>
<clause class="fontsize10" id="y14d61126-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/xiv" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="xiv">(xiv) </num><chapeau>The steps taken by the Department if an employee of the Department is suspected to be implicated in a suspicious death at a medical center of the Department, including—<page identifier="/us/stat/134/1021">134 STAT. 1021</page></chapeau><subclause class="fontsize10" id="y14d61127-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/xiv/I" style="-uslm-lc:I658128"><num class="fontsize10" style="-uslm-lc:emspace2" value="I">(I) </num><content>actions to remove or suspend that individual from patient care or temporarily reassign that individual and the speed at which that action occurs; and</content></subclause>
<subclause class="fontsize10" id="y14d61128-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/xiv/II" style="-uslm-lc:I658128"><num class="fontsize10" style="-uslm-lc:emspace2" value="II">(II) </num><content>steps taken to ensure that other medical centers of the Department and other non-Department medical centers are aware of the suspected role of the individual in a suspicious death.</content></subclause>
</clause>
<clause class="fontsize10" id="y14d61129-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/xv" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="xv">(xv) </num><content>In the case of the suspicious death of an individual while under care at a medical center of the Department, the methods used by the Department to inform the family members of that individual.</content></clause>
<clause class="fontsize10" id="y14d6112a-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/A/xvi" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="xvi">(xvi) </num><content>The policy of the Department for communicating to the public when a suspicious death occurs at a medical center of the Department.</content></clause>
</subparagraph>
<subparagraph class="fontsize10" id="y14d6112b-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/a/2/B" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="B">(B) </num><content>A description of any additional authorities or resources needed from Congress to implement any of the actions, changes to policy, or other matters included in the report required under paragraph (1)</content></subparagraph>
</paragraph>
</subsection>
<subsection class="firstIndent0 fontsize10" id="y14d6112c-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b" style="-uslm-lc:I658120"><num class="fontsize10" style="-uslm-lc:emspace2" value="b">(b) </num><heading class="fontsize10"><inline class="smallCaps">Report on Deaths at Louis A. Johnson Medical Center</inline>.—</heading><paragraph class="fontsize10" id="y14d6112d-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/1" style="-uslm-lc:I658122"><num class="fontsize10" style="-uslm-lc:emspace2" value="1">(1) </num><heading class="fontsize10"><inline class="smallCaps">In general</inline>.—</heading><chapeau>Not later than 60 days after the date on which the Attorney General indicates that any investigation or trial related to the suspicious deaths of veterans at the Louis A. Johnson VA Medical Center in Clarksburg, West Virginia, (in this subsection referred to as the “Facility”) that occurred during 2017 and 2018 has sufficiently concluded, the Secretary of Veterans Affairs shall submit to the Committee on Veterans’ Affairs of the Senate and the Committee on Veterans’ Affairs of the House of Representatives a report describing—</chapeau><subparagraph class="fontsize10" id="y14d6112e-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/1/A" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="A">(A) </num><content>the events that occurred during that period related to those suspicious deaths; and</content></subparagraph>
<subparagraph class="fontsize10" id="y14d6112f-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/1/B" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="B">(B) </num><content>actions taken at the Facility and throughout the Department of Veterans Affairs to prevent any similar reoccurrence of the issues that contributed to those suspicious deaths.</content></subparagraph>
</paragraph>
<paragraph class="fontsize10" id="y14d61130-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2" style="-uslm-lc:I658122"><num class="fontsize10" style="-uslm-lc:emspace2" value="2">(2) </num><heading class="fontsize10"><inline class="smallCaps">Elements</inline>.—</heading><chapeau>The report required by paragraph (1) shall include the following:</chapeau><subparagraph class="fontsize10" id="y14d61131-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/A" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="A">(A) </num><sidenote><p class="leftAlign firstIndent0 fontsize8" id="x14d61132-e826-11f0-bc57-ad3ac4b1618c" style="-uslm-lc:I658180">Timeline.</p></sidenote><chapeau>A timeline of events that occurred at the Facility relating to the suspicious deaths described in paragraph (1) beginning the moment those deaths were first determined to be suspicious, including any notifications to—</chapeau><clause class="fontsize10" id="y14d61133-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/A/i" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="i">(i) </num><content>leadership of the Facility;</content></clause>
<clause class="fontsize10" id="y14d61134-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/A/ii" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="ii">(ii) </num><content>leadership of the Veterans Integrated Service Network in which the Facility is located;</content></clause>
<clause class="fontsize10" id="y14d61135-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/A/iii" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="iii">(iii) </num><content>leadership at the central office of the Department; and</content></clause>
<clause class="fontsize10" id="y14d61136-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/A/iv" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="iv">(iv) </num><content>the Office of the Inspector General of the Department of Veterans Affairs.</content></clause>
</subparagraph>
<subparagraph class="fontsize10" id="y14d61137-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/B" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="B">(B) </num><content>A description of the actions taken by leadership of the Facility, the Veterans Integrated Service Network in which the Facility is located, and the central office of the Department in response to the suspicious deaths, including responses to notifications under subparagraph (A).<page identifier="/us/stat/134/1022">134 STAT. 1022</page></content></subparagraph>
<subparagraph class="fontsize10" id="y14d61138-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/C" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="C">(C) </num><content>A description of the actions, including root cause analyses, autopsies, or other activities that were conducted after each of the suspicious deaths.</content></subparagraph>
<subparagraph class="fontsize10" id="y14d61139-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/D" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="D">(D) </num><content>A description of the changes made by the Department since the suspicious deaths to procedures to control access within medical centers of the Department to controlled and non-controlled substances to prevent harm to patients.</content></subparagraph>
<subparagraph class="fontsize10" id="y14d6113a-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/E" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="E">(E) </num><content>A description of the changes made by the Department to its nationwide controlled substance and non-controlled substance policies as a result of the suspicious deaths.</content></subparagraph>
<subparagraph class="fontsize10" id="y14d6113b-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/F" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="F">(F) </num><content>A description of the changes planned or made by the Department to its video surveillance at medical centers of the Department to improve patient safety and quality of care in response to the suspicious deaths.</content></subparagraph>
<subparagraph class="fontsize10" id="y14d6113c-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/G" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="G">(G) </num><sidenote><p class="leftAlign firstIndent0 fontsize8" id="x14d6113d-e826-11f0-bc57-ad3ac4b1618c" style="-uslm-lc:I658180">Analysis.</p></sidenote><content>An analysis of the review of sentinel events conducted at the Facility in response to the suspicious deaths and whether that review was conducted consistent with policies and procedures of the Department.</content></subparagraph>
<subparagraph class="fontsize10" id="y14d6113e-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/H" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="H">(H) </num><content>A description of the steps the Department has taken or will take to improve the monitoring of the credentials of employees of the Department to ensure the validity of those credentials, including all employees that interact with patients in the provision of medical care.</content></subparagraph>
<subparagraph class="fontsize10" id="y14d6384f-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/I" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="I">(I) </num><content>A description of the steps the Department has taken or will take to monitor and mitigate the behavior of employee bad actors, including those who attempt to conceal their mistreatment of veteran patients.</content></subparagraph>
<subparagraph class="fontsize10" id="y14d63850-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/J" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="J">(J) </num><chapeau>A description of the steps the Department has taken or will take to enhance or create new monitoring systems that—</chapeau><clause class="fontsize10" id="y14d63851-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/J/i" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="i">(i) </num><content>automatically collect and analyze data from medical centers of the Department and monitor for warnings signs or unusual health patterns that may indicate a health safety or quality problem at a particular medical center; and</content></clause>
<clause class="fontsize10" id="y14d63852-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/J/ii" style="-uslm-lc:I658126"><num class="fontsize10" style="-uslm-lc:emspace2" value="ii">(ii) </num><content>automatically share those warnings with other medical centers of the Department, relevant Veterans Integrated Service Networks, and officials of the central office of the Department.</content></clause>
</subparagraph>
<subparagraph class="fontsize10" id="y14d63853-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/K" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="K">(K) </num><content>A description of the accountability actions that have been taken at the Facility to remove or discipline employees who significantly participated in the actions that contributed to the suspicious deaths.</content></subparagraph>
<subparagraph class="fontsize10" id="y14d63854-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/L" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="L">(L) </num><content>A description of the system-wide reporting process that the Department will or has implemented to ensure that relevant employees are properly reported, when applicable, to the National Practitioner Data Bank of the Department of Health and Human Services, the applicable State licensing boards, the Drug Enforcement Administration, and other relevant entities.<page identifier="/us/stat/134/1023">134 STAT. 1023</page></content></subparagraph>
<subparagraph class="fontsize10" id="y14d63855-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/M" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="M">(M) </num><content>A description of any additional authorities or resources needed from Congress to implement any of the recommendations or findings included in the report required under paragraph (1).</content></subparagraph>
<subparagraph class="fontsize10" id="y14d63856-e826-11f0-bc57-ad3ac4b1618c" identifier="/us/pl/116/212/s2/b/2/N" style="-uslm-lc:I658124"><num class="fontsize10" style="-uslm-lc:emspace2" value="N">(N) </num><content>Such other matters as the Secretary considers necessary.</content></subparagraph>
