which was referred to the <committee-name committee-id="HAG00">Committee on Agriculture</committee-name></action-desc></action><legis-type>A BILL</legis-type><official-title display="yes">To amend the Food, Conservation, and Energy Act of 2008 to reauthorize the Farm and Ranch Stress Assistance Network and establish a national agricultural crisis hotline, and for other purposes.</official-title></form><legis-body id="H59FB83845EFB480280AEA8C276B2E033" style="OLC"><section id="HB151AAF1A8514C68800B39249C4E1250" section-type="section-one"><enum>1.</enum><header>Short title</header><text display-inline="no-display-inline">This Act may be cited as the <quote><short-title>National Agricultural Crisis Hotline Act of 2023</short-title></quote>.</text></section><section id="H27C73036759F4A79B4D5E4CC6F8B680C"><enum>2.</enum><header>Findings</header><text display-inline="no-display-inline">Congress finds that—</text><paragraph id="H089E61B9CBC5425D83C6976E4EC47762"><enum>(1)</enum><text>the suicide rate for farmers is 3.5 times higher than the suicide rate of the general population;</text></paragraph><paragraph id="H1D7D15FFCA834CFDA19DB46EA6B0C8E4"><enum>(2)</enum><text>male farmers and ranchers have significantly higher rates of suicide (43.2 per 100,000) compared to the average rate of suicide across other professions (27.4 per 100,000);</text></paragraph><paragraph id="HB42D297924334FC7BA39490ACF754F75"><enum>(3)</enum><text>the percentage of adults ages 26 and older that live in completely rural areas has increased between 2019 and 2020, making completely rural areas the most afflicted county types with respect to alcohol abuse; and</text></paragraph><paragraph id="H5456BBF7DC534A939700B30E855AE6D1"><enum>(4)</enum><text>evidence suggests that the stigma surrounding mental health in farming and ranching communities has decreased but still plays a significant role in influencing behavior. </text></paragraph></section><section commented="no" display-inline="no-display-inline" id="HF80E8752FB934954A38B436A96D503F3"><enum>3.</enum><header>Farm and Ranch Stress Assistance Network</header><text display-inline="no-display-inline">Section 7522 of the Food, Conservation, and Energy Act of 2008 (<external-xref legal-doc="usc" parsable-cite="usc/7/5936">7 U.S.C. 5936</external-xref>) is amended—</text><paragraph commented="no" display-inline="no-display-inline" id="H5D68AE38D73B43C1855EE782531C0933"><enum>(1)</enum><text>in subsection (b)(1)—</text><subparagraph commented="no" display-inline="no-display-inline" id="H80A8FE68C90A494E80641234AFCF5AA6"><enum>(A)</enum><text>by redesignating subparagraphs (C) and (D) as subparagraphs (D) and (E), respectively; and</text></subparagraph><subparagraph commented="no" display-inline="no-display-inline" id="H53E220F8DDCD4ABFB1D113CFE3F83A1C"><enum>(B)</enum><text>by inserting after subparagraph (B) the following:</text><quoted-block style="OLC" display-inline="no-display-inline" id="HD37111438C384C8087D522872573BDD6"><subparagraph commented="no" display-inline="no-display-inline" id="H89301C0119624981BA4484C08B02010D"><enum>(C)</enum><text>mental health training for agriculture professionals (including farm loan officers, extension agents, and State department of agriculture staff);</text></subparagraph><after-quoted-block>;</after-quoted-block></quoted-block></subparagraph></paragraph><paragraph id="H1659ECF815CB422BA934B7AE7258B874"><enum>(2)</enum><text>by redesignating subsections (d) through (f) as subsections (e) through (g), respectively;</text></paragraph><paragraph commented="no" display-inline="no-display-inline" id="H234BC3724E344FEABC63E2BEABDAF332"><enum>(3)</enum><text display-inline="yes-display-inline">by inserting after subsection (c) the following:</text><quoted-block style="OLC" display-inline="no-display-inline" id="H24955A47511B41B5AD40FB6328BC7420"><subsection id="H8DC56412EA9B41FD80ABEE594A3F926E"><enum>(d)</enum><header>National agricultural crisis hotline</header><paragraph commented="no" display-inline="no-display-inline" id="HE1D92D6FCA3A46C78F5239213746F27C"><enum>(1)</enum><header>In general</header><text display-inline="yes-display-inline">As part of the Farm and Ranch Stress Assistance Network established under subsection (a), the Secretary shall establish, and maintain directly or by grant or contract, a national agricultural crisis hotline (referred to in this subsection as the <quote>national agricultural crisis hotline</quote>) to provide emotional support, information, suicide crisis intervention, mental health and substance use disorder, and agricultural support resources to—</text><subparagraph commented="no" display-inline="no-display-inline" id="H05BE4524BE7541F4858A272B61A298E4"><enum>(A)</enum><text display-inline="yes-display-inline">farmers, farm managers, farm workers, and ranchers affected by mental health and substance use disorders; and </text></subparagraph><subparagraph commented="no" display-inline="no-display-inline" id="H3AE58AFDF0264AB0A9D2DE99224CAEF7"><enum>(B)</enum><text display-inline="yes-display-inline">families or household members of those farmers, farm managers, farm workers, and ranchers.</text></subparagraph></paragraph><paragraph id="H4D5F35378E744BD095D5546D472EE999"><enum>(2)</enum><header>Requirements</header><text>The national agricultural crisis hotline shall—</text><subparagraph id="H6CC0F59CA03447BD84F8B8D0B3317E6A"><enum>(A)</enum><text>provide 24/7 real-time live assistance by voice and through a mobile texting application; </text></subparagraph><subparagraph id="H3C086D0053264584981A28DCB3BEA872"><enum>(B)</enum><text>be staffed 24/7 in accordance with paragraph (4);</text></subparagraph><subparagraph commented="no" display-inline="no-display-inline" id="H5CBA25B4C47B48EB8509EA692133B42B"><enum>(C)</enum><text display-inline="yes-display-inline">provide referral services to meet the needs of family and household members of farmers, ranchers, and related professionals;</text></subparagraph><subparagraph id="H3327B75DF52D481A89D701E834571576"><enum>(D)</enum><text>offer 24-hour follow-up calls to callers; </text></subparagraph><subparagraph id="HD4CEDC067BB346818A59C82E6C72D611"><enum>(E)</enum><text>provide standardized translation services that meet the needs of the agricultural population; and</text></subparagraph><subparagraph id="HDABFA5531EF341578EA0963AD7F91E1C"><enum>(F)</enum><text>comply with all Federal, State, and local laws, including the Health Insurance Portability and Accountability Act of 1996 (<external-xref legal-doc="public-law" parsable-cite="pl/104/191">Public Law 104–191</external-xref>; 110 Stat. 1936).</text></subparagraph></paragraph><paragraph id="HFBA4F0613D874BB3925488CA459D34BF"><enum>(3)</enum><header>Data collection</header><text>The national agricultural crisis hotline shall collect the following data:</text><subparagraph id="HAF3621E7CDEA419F86EE87995938FD51"><enum>(A)</enum><text>Call data, including total calls, calls queued, calls answered, calls dropped, and average speed of answer.</text></subparagraph><subparagraph id="H9D9D650B762549F5918E9FD09DBA27AE"><enum>(B)</enum><text>Rescue data, including next steps suggested and information provided to the caller.</text></subparagraph><subparagraph id="H6B6DEA2F2C884859BAE8851F497CC2B1"><enum>(C)</enum><text>Follow-up calls.</text></subparagraph><subparagraph id="H051CCA13A15F47898616F5242080044D"><enum>(D)</enum><text>Caller information, including—</text><clause id="H833E9F99D82740DD971B948D417F3171"><enum>(i)</enum><text>whether the caller is calling for the first time or is a repeat caller;</text></clause><clause id="H2DD1D4F423734E31991B167966CE76A8"><enum>(ii)</enum><text>the living situation of the caller; and</text></clause><clause commented="no" display-inline="no-display-inline" id="HA0A18F2ABE7947838EE11D47ACE2BBE2"><enum>(iii)</enum><text>demographic information of the caller.</text></clause></subparagraph><subparagraph id="HF08B344BDD5A474DAFE753144D088356"><enum>(E)</enum><text>Reason for call and caller needs and concerns.</text></subparagraph><subparagraph id="H9310CF250DDC419C968C07C1331E5B08"><enum>(F)</enum><text>Referrals provided, including resources provided and strategies employed with respect to addressing the needs and concerns of callers.</text></subparagraph></paragraph><paragraph id="H196E095C9C624E5E994397EBB1E0F7FB"><enum>(4)</enum><header>Staffing</header><text>Call responders and supervisors of call responders operating the national agricultural crisis hotline shall—</text><subparagraph id="H4CBC83E8FDB74B239DA06E362C59FC95"><enum>(A)</enum><text>be crisis intervention specialists, licensed health care professionals, or licensed mental health professionals who—</text><clause id="H2971A7599AD449869DF5DA5007637D27"><enum>(i)</enum><text>are trained in providing culturally and linguistically appropriate support;</text></clause><clause id="H2B011BFD90AB48EC95C42806974C8E22"><enum>(ii)</enum><text>have awareness training with respect to farm stress and the farming identity; and</text></clause><clause id="HAAF4AB853F2A4F37AA4ECDA14EF4C9EB"><enum>(iii)</enum><text>have credentials that enable them to provide evidence-based suicide intervention hotline care, including accreditation from the American Association of Suicidology and the Alliance of Information and Referral Systems; and</text></clause></subparagraph><subparagraph id="H0698C643D6854427A46334FD41302023"><enum>(B)</enum><text>meet training requirements developed by the Secretary or the entity maintaining the hotline, as applicable, which, at a minimum, shall include—</text><clause id="H7DF68B6A28414514920EE94DC56062BC"><enum>(i)</enum><text>at least 80 hours of interactive training and content, which shall include—</text><subclause commented="no" display-inline="no-display-inline" id="HD6FFA2F4D77B4395B8059AC29B39A834"><enum>(I)</enum><text display-inline="yes-display-inline">elements of applied suicide intervention skills training;</text></subclause><subclause commented="no" display-inline="no-display-inline" id="HE6BDC4F2C83A46CFADC80907C4F629E0"><enum>(II)</enum><text display-inline="yes-display-inline">information and training on providing trauma informed support; and</text></subclause><subclause commented="no" display-inline="no-display-inline" id="H9759F765CC874BBAB3DC850B1C07D6AF"><enum>(III)</enum><text display-inline="yes-display-inline">training and content on other relevant clinical topics;</text></subclause></clause><clause id="H56A7CC3F2E2C43DEAF946244E99C63CD"><enum>(ii)</enum><text>foundational agriculture mental stress training that includes metrics that demonstrate competency;</text></clause><clause id="HA7E5D1C06C894AD9895F746F57C7E0FE"><enum>(iii)</enum><text>a minimum of 24 hours of direct supervised shifts before answering calls independently; and</text></clause><clause id="HDE1239B035B74DA184A4F08CA255A8BD"><enum>(iv)</enum><text>ongoing training on specialized topics on at least a quarterly basis.</text></clause></subparagraph></paragraph><paragraph id="HAD237F76F7EB4B288A3FDFF6AD284E61" commented="no" display-inline="no-display-inline"><enum>(5)</enum><header>Partnerships</header><subparagraph commented="no" display-inline="no-display-inline" id="HBAE99D14E7D24E8C9D2D493626FBDBFC"><enum>(A)</enum><header display-inline="yes-display-inline">Existing hotlines</header><text display-inline="yes-display-inline">The Secretary or an entity receiving a grant or contract under paragraph (1), as applicable, shall form partnerships between the national agricultural crisis hotline and existing farm telephone helplines and websites.</text></subparagraph><subparagraph commented="no" display-inline="no-display-inline" id="H8497B8F77445428A94690E089C324064"><enum>(B)</enum><header>USDA and other Federal programs</header><text>The Secretary shall coordinate with—</text><clause commented="no" display-inline="no-display-inline" id="H0A2BEF6773374212A752324584840090"><enum>(i)</enum><text display-inline="yes-display-inline">the Federal Office of Rural Health Policy and offices of the Department of Agriculture that administer financial assistance programs to farmers and ranchers to provide information about the national agricultural crisis hotline to participants of those programs; and</text></clause><clause commented="no" display-inline="no-display-inline" id="H04044CC1910C4D5AAF559FD90B1D2963"><enum>(ii)</enum><text>the Substance Abuse and Mental Health Services Administration to refer agriculture related calls received through the 988 Suicide and Crisis Lifeline to the national agricultural crisis hotline.</text></clause></subparagraph></paragraph><paragraph commented="no" display-inline="no-display-inline" id="HBA578039628147B0B7D833472C72973D"><enum>(6)</enum><header>Directory</header><text>The Secretary or an entity receiving a grant or contract under paragraph (1), as applicable, shall develop, and periodically update, a directory of organizations to which staff of the national agricultural crisis hotline can make referrals.</text></paragraph><paragraph id="HD264E69070EA4C5FA663215ABCF806DC"><enum>(7)</enum><header>Outreach</header><text>The Secretary shall, in consultation with the Department of Health and Human Services, the Substance Abuse and Mental Health Services Administration, any other relevant Federal agencies, and State departments of health and agriculture, conduct a public awareness campaign for the national agricultural crisis hotline. </text></paragraph><paragraph id="H4A617A3EA05E4385800878576F6B1B36"><enum>(8)</enum><header>Report</header><text>The Secretary shall submit to Congress and make publicly available an annual report on the effectiveness of the national agricultural crisis hotline.</text></paragraph></subsection><after-quoted-block>; and</after-quoted-block></quoted-block></paragraph><paragraph commented="no" display-inline="no-display-inline" id="H7C32B535CC5C49E28989D66B3D8F5540"><enum>(4)</enum><text>in subsection (e) (as so redesignated), by striking <quote>$10,000,000 for each of fiscal years 2019 through 2023</quote> and inserting <quote>$19,000,000 for each of fiscal years 2024 through 2028, of which $4,000,000 each fiscal year shall be used to carry out subsection (d)</quote>.</text></paragraph></section></legis-body></bill>
