			 interprofessional geriatric education and training to develop a
			 geriatric-capable workforce, improving health outcomes for a growing and
			 diverse aging American population and their families, and for other
			 purposes.</official-title></form>
	<legis-body display-enacting-clause="yes-display-enacting-clause">
 <section id="S1" section-type="section-one"><enum>1.</enum><header>Short title</header><text display-inline="no-display-inline">This Act may be cited as the <quote><short-title>Geriatrics Workforce Improvement Act</short-title></quote>.</text> </section><section id="id45673e4ad75f46f69b9a0dc56fad3f09"><enum>2.</enum><header>Findings</header><text display-inline="no-display-inline">Congress finds the following:</text>
 <paragraph id="idad0d0b6f2b064feb92b8f8c63b6888bd"><enum>(1)</enum><text>In 2016, 46,000,000 adults age 65 or older lived in the United States, and this number is expected to double to over 98,000,000 by 2060. Three out of four older adults are expected to have multiple chronic conditions, which increases the complexity of care needed and increases medical expenditures.</text>
 </paragraph><paragraph id="id15221041C1804E1DB9193158106CB616"><enum>(2)</enum><text>Thirty percent of older adults in the United States, about 14,000,000 individuals, are projected to need specialized geriatric care by 2030. That care will require at least 20,000 geriatricians and even more geriatric-trained health professionals. There are nearly 1,000,000 physicians in the United States, less than 7,300 of whom are board-certified geriatricians. There are 3,600,000 nurses in the United States, of whom, less than 1 percent are registered nurses certified in geriatrics and less than 3 percent are advanced practice nurses certified in geriatrics. There are similarly few professionals certified in geriatrics among professionals in social work, pharmacy, psychiatry, and the allied health disciplines.</text>
 </paragraph><paragraph id="id5dcb324ba4054a3da55fa99a0317e4c1"><enum>(3)</enum><text>Health professionals trained in geriatrics understand the unique health needs and complex care challenges associated with aging. Outcomes associated with interprofessional geriatric teams include improved health-related quality of life, fewer emergency room visits, fewer hospital admissions and, in the case of hospitalization, shorter length of stay and lower costs per admission.</text>
 </paragraph><paragraph id="id100687abc7a3409ea44017b57ce909c0"><enum>(4)</enum><text>Two federally funded initiatives have historically aimed to reduce the widening gap between the numbers of older adults and health professionals trained in geriatrics: The Geriatrics Workforce Enhancement Program and the Geriatrics Academic Career Award.</text>
 </paragraph></section><section id="id5818e80c15624e81b546a3f3a2d89575"><enum>3.</enum><header>Purpose</header><text display-inline="no-display-inline">It is the purpose of this Act to develop the next generation of geriatric scientists and innovators, improving health outcomes and care delivery for older adults. Together, the Geriatrics Workforce Enhancement Program and the Geriatrics Academic Career Award develop a workforce capable of providing complex, high-quality care that improves health outcomes and saves valuable resources by reducing unnecessary costs for a growing and diverse aging population.</text>
 </section><section id="id60B24B0AA297420C979750617536253B"><enum>4.</enum><header>Education and training relating to geriatrics</header><text display-inline="no-display-inline">Section 753 of the Public Health Service Act (<external-xref legal-doc="usc" parsable-cite="usc/42/294c">42 U.S.C. 294c</external-xref>) is amended to read as follows:</text> <quoted-block display-inline="no-display-inline" id="id92b44ebdebe2477887701ffc52de6cfa" style="OLC"> <section id="id2f8702c2a7624d0c9a5fe99cfa0419aa"><enum>753.</enum><header>Education and training relating to geriatrics</header> <subsection id="id7e4845e76b0344e9a7d7d3fb025fa139"><enum>(a)</enum><header>Geriatrics workforce enhancement program</header> <paragraph id="id107757b6fb0d4380a261a27f5fabf38a"><enum>(1)</enum><header>In general</header><text>The Secretary shall award grants under this subsection to entities described in paragraph (1), (3), or (4) of section 799B, section 801(2), or section 865(d), or other health professions schools or programs approved by the Secretary, for the establishment or operation of Geriatrics Workforce Enhancement Programs that meet the requirements of paragraph (2).</text>
						</paragraph><paragraph id="idfdb1dba8e03a4aa6bda44798f4f0f289"><enum>(2)</enum><header>Requirements</header>
 <subparagraph id="id1A44CFBBDEFA4E5D856154FDF06E4171"><enum>(A)</enum><header>In general</header><text>A Geriatrics Workforce Enhancement Program meets the requirements of this paragraph if such program supports the development of a health care workforce that maximizes patient and family engagement and improves health outcomes for older adults by integrating geriatrics with primary care and other appropriate specialties. Special emphasis shall be placed on providing the primary care workforce with the knowledge and skills to care for older adults and collaborating with community partners to address gaps in health care for older adults through individual-, system-, community-, and population-level changes.</text>
 </subparagraph><subparagraph id="idA6894E3F9E1640F2B63920260884ADAD"><enum>(B)</enum><header>Programmatic focus</header><text>Areas of programmatic focus for a program meeting the requirements of this paragraph may include the following:</text>
 <clause id="id6b031bb0af154c4a84ffb87828778810"><enum>(i)</enum><text>Transforming clinical training environments into integrated geriatrics and primary care delivery systems to ensure trainees are well prepared to practice in and lead such systems.</text>
 </clause><clause id="id6e4ecafb53994926952dbaaa954f5021"><enum>(ii)</enum><text>Developing providers from multiple disciplines and specialties to work interprofessionally to assess and address the needs and preferences of older adults and their families and caregivers at the individual, community, and population levels.</text>
 </clause><clause id="idaa7b4bffbb6042b1a60eaf971f2ff912"><enum>(iii)</enum><text>Creating and delivering community-based programs that will provide older adults and their families and caregivers with education and training to improve health outcomes and the quality of care for such adults.</text>
 </clause><clause id="idc5e77296bd4a473e9fd4574190fd57ec"><enum>(iv)</enum><text>Providing education on Alzheimer's disease and related dementias to families and caregivers of older adults, direct care workers, and health professions students, faculty, and providers.</text>
 </clause></subparagraph></paragraph><paragraph id="id0a7e1bb1909844cdb73f1a38e67f8e70"><enum>(3)</enum><header>Duration</header><text>The Secretary shall award grants under paragraph (1) for a period not to exceed 5 years.</text> </paragraph><paragraph id="ide739d269088242b391aa68fd2e3acb6e"><enum>(4)</enum><header>Applications</header><text>To be eligible to receive a grant under paragraph (1), an entity described in such paragraph shall submit to the Secretary an application at such time, in such manner, and containing such information as the Secretary may require, including the specific measures the applicant will use to demonstrate that the project is improving the quality of care provided to older adults in the applicant’s region, which may include—</text>
 <subparagraph id="id2BDAE66C23764C7295EC498C76DB894B"><enum>(A)</enum><text>improvements in access to care provided by a health professional with training in geriatrics or gerontology;</text>
 </subparagraph><subparagraph id="idD510E1159F6F4988A06B6DA41A1C3F86"><enum>(B)</enum><text>improvements in family caregiver capacity to care for older adults;</text> </subparagraph><subparagraph id="id19BD219D00A44E10A89CF0E663192032"><enum>(C)</enum><text>patient outcome data demonstrating an improvement in older adult health status or care quality; and</text>
 </subparagraph><subparagraph id="id0B97FA727A204836816DE3C05C46C5A1"><enum>(D)</enum><text>reports on how the applicant will implement specific innovations with the target audience to improve older adults health status or the quality of care.</text>
							</subparagraph></paragraph><paragraph id="idB830729D18834B838103F40909B1B29D"><enum>(5)</enum><header>Program requirements</header>
 <subparagraph id="id9D39E4964E5F47AFA1275B52E639243A"><enum>(A)</enum><header>In general</header><text>In awarding grants under paragraph (1), the Secretary—</text> <clause id="idce4db8ed7c02476ab87b988e8786db4a"><enum>(i)</enum><text>shall ensure an equitable geographic distribution of grant recipients based on State and regional aging demographics;</text>
 </clause><clause id="ide6a64c85400d4216b0badd9977dba04f"><enum>(ii)</enum><text>shall give priority to programs that demonstrate coordination with other programmatic efforts funded under this program or other public or private entities;</text>
 </clause><clause id="id9B78F9A7206E4505BC8EF9D8DF6B26AF"><enum>(iii)</enum><text>shall give priority to applicants with programs or activities should substantially benefit rural, underserved, or Native American populations of older adults; and</text>
 </clause><clause id="id9f1ca2a632dc4b7f940b4c24b3733806"><enum>(iv)</enum><text>may give priority to any program that—</text> <subclause id="id9c13c28a54954a68a34d8c72a8a3ef3e"><enum>(I)</enum><text>integrates geriatrics and gerontology into primary care practice, especially with respect to medical, dental, and psychosocial care, elder abuse, pain management, and advance care planning;</text>
 </subclause><subclause id="idd452c8250c0d42b680240387362465bf"><enum>(II)</enum><text>offers courses to infuse core geriatric principles of care into other specialties across care settings, including practicing clinical specialists, health care administrators, faculty without backgrounds in geriatrics, students from all health professions, as approved by the Secretary, to improve knowledge and clinical skills for the care of older adults;</text>
 </subclause><subclause id="idE0139EACB83745B6B4029900F8F45496"><enum>(III)</enum><text>emphasizes integration into existing service delivery locations and care across settings, including primary care clinics, medical homes, Federally qualified health centers, ambulatory care clinics, age-friendly health systems, critical access hospitals, emergency care, assisted living and nursing facilities, and home- and community-based services, including senior day care;</text>
 </subclause><subclause id="id42C01C67248046CE856625D50EFBC86F"><enum>(IV)</enum><text>supports the training and retraining of faculty, preceptors, primary care providers, and other direct care providers to increase their knowledge of geriatrics and gerontology;</text>
 </subclause><subclause id="idCC2B1F33085B4ACAA556BCFDC202776C"><enum>(V)</enum><text>emphasizes education and engagement of family caregivers on disease self-management, medication management, and stress-reduction strategies for older adults;</text>
 </subclause><subclause id="id23F98165532C468C9539AA071536A0D2"><enum>(VI)</enum><text>provides training to the health care workforce on disease self-management, medication management, stress-reduction strategies, and social determinants of health in older adults; or</text>
 </subclause><subclause id="id07028ADDBB484D6AA2342B4694B5D73E"><enum>(VII)</enum><text>proposes to conduct outreach to communities that have a shortage of geriatric workforce professionals.</text>
 </subclause></clause></subparagraph><subparagraph id="id2283BF1BF8084934B97DA31A3B574841"><enum>(B)</enum><header>Special consideration</header><text>In awarding grants under paragraph (1), particularly with respect to awarding, in fiscal year 2020, any amount appropriated for such fiscal year for purposes of carrying out this subsection that is in excess of the amount appropriated for the most recent previous fiscal year for which appropriations were made such purposes, the Secretary shall give special consideration to entities that operate—</text>
 <clause id="id5BC55108691F4DCA9978C3123CD1BADF"><enum>(i)</enum><text>in communities that have a shortage of geriatric workforce professionals; and</text>
 </clause><clause id="id12E10510788246C8A5014F45052FD6AA"><enum>(ii)</enum><text>in States in which no entity has previously received an award under such paragraph (including under subsection (a)(1) of this section as in effect before the date of enactment of the <short-title>Geriatrics Workforce Improvement Act</short-title>).</text>
 </clause></subparagraph></paragraph><paragraph id="idA91BBB2F9702462FB4D02AD132ED813A"><enum>(6)</enum><header>Award amounts</header><text>Awards under paragraph (1) shall be in an amount determined by the Secretary. Entities that submit applications under this subsection that describe a plan for providing geriatric education and training for home health workers and family caregivers are eligible to receive $100,000 per year more than entities that do not include a description of such a plan.</text>
						</paragraph><paragraph id="id91587BA446404D23A05FAB4D71193BA0"><enum>(7)</enum><header>Reporting</header>
 <subparagraph id="idEB174C5CCCC44625B55D8E24534B4719"><enum>(A)</enum><header>Reports from entities</header><text>Each entity awarded a grant under paragraph (1) shall submit an annual report to the Secretary on the financial and programmatic performance under such grant, which may include factors such as the number of trainees, the number of professions and disciplines, the number of partnerships with health care delivery sites, the number of faculty and practicing professionals who participated in continuing education programs, and other factors, as the Secretary may require.</text>
							</subparagraph><subparagraph id="id67605788AF31467993B990F1EA900B0F"><enum>(B)</enum><header>Reports to Congress</header>
								<clause id="id9338F47E49C7464E9CB115BD2215B89B"><enum>(i)</enum><header>In general</header>
 <subclause id="idF591E01137944DA39A52FA8B4C14BE22"><enum>(I)</enum><header>Annual report</header><text>At the end of each year in which the Secretary awards grants under this subsection, the Secretary shall submit to Congress a report that provides a summary of the financial and programmatic performance of such grants, which may include factors such as the number trainees, the number of professions and disciplines, the number of partnerships with health care delivery sites, the number of faculty and practicing professionals who participated in continuing education programs, and other factors that assess the impact of the program under this subsection on the health status of older adults.</text>
 </subclause><subclause id="id43CB017A8DAB406B917E3917BF09150A"><enum>(II)</enum><header>Summary reports</header><text>Not later than 1 year after the completion of each 5-year grant term under this subsection, the Secretary shall submit to Congress a report—</text>
 <item id="idD4ECBD306BFE4A52801A36C26609FAC4"><enum>(aa)</enum><text>that provides a summary of the financial and programmatic performance of the funded grants and a summary of other factors that assess the impact of the program under this subsection on the health status of older adults, quality of care for older adults, and the knowledge and skills of the Nation’s health care workforce to care for older adults; and</text>
 </item><item id="idEC5BB0E4385C4DEA94620B165C79D322"><enum>(bb)</enum><text>which may include factors such as the number of trainees, the number of professions and disciplines, the number of partnerships with health care delivery sites, and the number of faculty and practicing professionals who participated in continuing education program.</text>
 </item></subclause></clause><clause id="id132C2DC81C6E43A9BD9322B850CC9CC9"><enum>(ii)</enum><header>Public availability</header><text>The Secretary shall make each report submitted under clause (i), and supporting data, publicly available in an accessible format on the internet website of the Health Resources and Services Administration.</text>
 </clause></subparagraph></paragraph><paragraph id="idcd9e0d6602974816835512253b4f3286"><enum>(8)</enum><header>Authorization of appropriations</header><text>For purposes of carrying out this subsection, in addition to any other funding available for such purpose, there is authorized to be appropriated $45,000,000 for each of fiscal years 2020 through 2024.</text>
						</paragraph></subsection><subsection id="idcae7b402722a410c99e728e157d46f28"><enum>(b)</enum><header>Geriatric Academic Career Awards</header>
 <paragraph id="id330f226373b94ebd9bbbf133d79be14f"><enum>(1)</enum><header>Establishment of program</header><text>The Secretary shall establish a program to provide geriatric academic career awards to eligible entities applying on behalf of eligible individuals to promote the career development of such individuals as academic geriatricians or other academic geriatrics health professionals.</text>
						</paragraph><paragraph id="id1cc57c9baf81412cb6142ed8a57b9fe3"><enum>(2)</enum><header>Eligibility</header>
 <subparagraph id="id8FEEE337C3F5476EAFDAA99D898ABE5A"><enum>(A)</enum><header>Eligible entity</header><text>For purposes of this subsection, the term <term>eligible entity</term> means—</text> <clause id="idAA8477FCE03D4245A1A0520EA3C9D44F"><enum>(i)</enum><text>an entity described in paragraph (1), (3), or (4) of section 799B or section 801(2); or</text>
 </clause><clause id="idB377C94DAFE54BEEB9044B5C2041A042"><enum>(ii)</enum><text>another accredited health professions school or graduate program approved by the Secretary.</text> </clause></subparagraph><subparagraph id="id51308CCCA44142BBA06C9C00D55B1177"><enum>(B)</enum><header>Eligible individual</header><text>For purposes of this subsection, the term <term>eligible individual</term> means an individual who—</text>
 <clause id="ida0a9fe762b274254a8e1668e232a8fb0"><enum>(i)</enum><subclause commented="no" display-inline="yes-display-inline" id="id77EB0162601D49D18C8C7EE2D80F7016"><enum>(I)</enum><text>is board certified or board eligible in internal medicine, family practice, psychiatry, or licensed dentistry, or has completed required training in a discipline and is employed in an accredited health professions school or graduate program that is approved by the Secretary; or</text>
 </subclause><subclause id="idda707fce41f54009968442cbfbcd4d1c" indent="up1"><enum>(II)</enum><text>has completed an approved fellowship program in geriatrics or gerontology, or has completed specialty training in geriatrics or gerontology as required by the discipline and any additional geriatrics or gerontology training as required by the Secretary; and</text>
 </subclause></clause><clause id="idb5f4246df3a74625ae7358cf1e971a49"><enum>(ii)</enum><text>has a junior, nontenured, faculty appointment at an accredited health professions school or graduate program in geriatrics or a geriatrics health profession such as psychology, pharmacy, nursing, social work, dentistry, public health, allied health, health care administration, or another health discipline, as determined by the Secretary.</text>
 </clause></subparagraph></paragraph><paragraph id="idf3f3dc1be1e84a4cac78f4de1e589b7e"><enum>(3)</enum><header>Application requirements</header><text>In order to receive an award under paragraph (1), an eligible entity, on behalf of eligible individuals, shall—</text>
 <subparagraph id="id3d71c7073f234c5fb256ac55eee57ab4"><enum>(A)</enum><text>submit to the Secretary or a designee an application, at such time, in such manner, and containing such information as the Secretary may require;</text>
 </subparagraph><subparagraph id="idd2f3711ff7c342bda0adf6f33f80536a"><enum>(B)</enum><text>provide, in such form and manner as the Secretary may require, assurances that the eligible individual on whose behalf an application was submitted will meet the service requirement described in paragraph (6); and</text>
 </subparagraph><subparagraph id="id1e0ca94c73f24c89b89a41bc0ebb03d3"><enum>(C)</enum><text>provide, in such form and manner as the Secretary may require, documented commitment from such school or program to spend 50 percent of the individual's time that is supported by the award on teaching and developing skills in interprofessional education in geriatrics.</text>
 </subparagraph></paragraph><paragraph id="idbc828b27deaa49279d7fa2f0673be5ba"><enum>(4)</enum><header>Equitable distribution</header><text>In making awards under this subsection, the Secretary shall ensure that awards are equitably distributed, including among rural or underserved populations across the various geographical regions of the United States.</text>
						</paragraph><paragraph id="idd7eef0ec05424dc0b1475aa6a7ed9fe8"><enum>(5)</enum><header>Amount and duration</header>
 <subparagraph id="idab6bbc999fcc4205afa7c64ba6fda226"><enum>(A)</enum><header>Amount</header><text>The amount of an award under this subsection for an eligible individual for each fiscal year of such award shall be the lesser of—</text>
 <clause id="idF3F45618623B4C028022FF86D75BE6F1"><enum>(i)</enum><text>50 percent of the eligible individual’s annual full-time salary for such fiscal year; or</text> </clause><clause id="id8C121E9101DC47C4AF645753D0BF668C"><enum>(ii)</enum><text>$90,000.</text>
 </clause></subparagraph><subparagraph id="idfbe9b04f56204a8c8b3acb5fa31c7b36"><enum>(B)</enum><header>Duration</header><text>The Secretary shall make awards under paragraph (1) for a period not to exceed 5 years.</text> </subparagraph></paragraph><paragraph id="id00d0d504b7124d6e8c3e5be211c2987f"><enum>(6)</enum><header>Service requirement</header><text>An individual who receives an award under this subsection shall provide training in clinical geriatrics or gerontology, including the training of interprofessional teams of health care professionals. The provision of such training shall constitute at least 50 percent of the obligations of such individual under the award.</text>
 </paragraph><paragraph id="id337f968cc23d438ea6e17bde296c45f2"><enum>(7)</enum><header>Authorization of appropriations</header><text>There is authorized to be appropriated $6,000,000 for each of fiscal years 2020 through 2024 for purposes of carrying out this subsection.</text>
 </paragraph></subsection><subsection id="id3FD7CC9D95C44C67A3CB29E6F38BFAA2"><enum>(c)</enum><header>Inapplicability</header><text>Section 791(a) shall not apply with respect to grants under subsection (a) or awards under subsection (b).</text></subsection></section><after-quoted-block>.</after-quoted-block></quoted-block></section></legis-body></bill>
