			 of Health and Human Services to develop guidelines on reducing health
			 disparities, and for other purposes.</official-title>

		<section id="H20532552CA8E4F52A3DB1F45FEEAC85F" section-type="section-one"><enum>1.</enum><header>Short title</header><text display-inline="no-display-inline">This Act may be cited as the <quote><short-title>Reducing Disparities Using Care Models and Education Act of 2014</short-title></quote>.</text>
		</section><section id="H2F1B5FBE5BE04B0A9D39B1F4957B0EF4"><enum>2.</enum><header>Findings</header><text display-inline="no-display-inline">The Congress finds as follows:</text>
			<paragraph id="H7930FBB6D63E4902AA3ADA19CD395E27"><enum>(1)</enum><text>The infant death rate among African-Americans is more than double that of Whites.</text>
			</paragraph><paragraph id="H6179C772C45847F8B3434083F16BF4EE"><enum>(2)</enum><text>The death rate for all cancers is 30 percent higher for African-Americans than for Whites; for
			 prostate cancer, it is more than double that for Whites.</text>
			</paragraph><paragraph id="HB1C219D609B34CE08A792F67FFDF37BA"><enum>(3)</enum><text>Black women have a higher death rate from breast cancer despite having a mammography screening rate
			 that is nearly the same as the rate for White women.</text>
			</paragraph><paragraph id="H28FD1499C3C8442FA54AF53B4ABD061E"><enum>(4)</enum><text>Diabetes incidence is highest among Native Americans, at 15.9 percent, followed by 13.2 percent for
			 African-Americans, 12.8 percent for Hispanics, 9.0 percent for Asians, and
			 7.6 percent for Whites.</text>
			</paragraph><paragraph id="H68A0CF83D9B846AF94D14E41F8DE54D2"><enum>(5)</enum><text>New cases of hepatitis and tuberculosis are higher in Asians and Pacific Islanders living in the
			 United States than in Whites.</text>
			</paragraph><paragraph id="HDDB47A2495B24D52A4CFA2F35CCA390F"><enum>(6)</enum><text>Individuals in same-sex couples were more likely than individuals in different-sex couples to
			 report a delay in getting necessary prescriptions.</text>
			</paragraph><paragraph id="H2D7049E0C13E42E29996A67C3F1BF9BE"><enum>(7)</enum><text>Infants born to Black women are 1.5 to 3 times more likely to die than those born to women of other
			 races or ethnicities, and American Indian and Alaska Native infants die
			 from sudden infant death syndrome (SIDS) at nearly 2.5 times the rate of
			 White infants.</text>
			</paragraph><paragraph id="HFA9A676B6BA14634A4F281807CCD7B68"><enum>(8)</enum><text display-inline="yes-display-inline">Low-income children have higher rates of mortality (even with the same condition), have higher
			 rates of disability, and are more likely to have multiple conditions.</text>
			</paragraph><paragraph id="HAFA5F3CB302B482E88E665C5B263B605"><enum>(9)</enum><text display-inline="yes-display-inline">White children are half as likely as Black and Latino children not to be in excellent or very good
			 health.</text>
			</paragraph><paragraph id="H66E29617E45B4B61AC322F333C88F52E"><enum>(10)</enum><text>As of 2012, 38.9 percent of United States adults were obese, with the highest rate among
			 African-Americans at 47.9 percent, followed by Hispanics at 42.5 percent,
			 Whites
			 at 32.6 percent, and Asians at 10.8 percent.</text>
			</paragraph><paragraph id="H5DA056AA12344389BA96C6A19650E6B8"><enum>(11)</enum><text>The risk of stroke is twice as high for African-Americans as for Whites, and African-Americans are
			 more likely to die of stroke. Other ethnic minorities also have higher
			 risk than Whites. Overall, strokes are most prevalent in the Southeast
			 United States, and less so in the Northeast.</text>
			</paragraph><paragraph id="HCD6E271DC3F84C2F80C217121D1B4853"><enum>(12)</enum><text>African-Americans accounted for 44 percent of all those infected with HIV, despite being only 12
			 percent of the United States population.</text>
			</paragraph><paragraph id="H18DBB2AE819E4D02A019DF29ADCE0A5F"><enum>(13)</enum><text>Black men who have sex with men (MSM) ages 13 to 24 had the most new infections among youth.</text>
			</paragraph><paragraph id="H5363E5E37C5746C4B53018F66435D8F2"><enum>(14)</enum><text>One study found that among heterosexuals living in the same urban community, those below the
			 poverty line were twice as likely to contract human immunodeficiency virus
			 (HIV).</text>
			</paragraph><paragraph id="H57791DA287E847B0AF27A0BC4B6FC46A"><enum>(15)</enum><text>Persons with less than a high school diploma (6.7 percent) and high school graduates (4.0 percent)
			 were more likely to report major depression than those with at least some
			 college education (2.5 percent).</text>
			</paragraph><paragraph id="H9CF6A557C1284C268CB3A72F355EFBF4"><enum>(16)</enum><text>Only about 10 percent of physicians practice in rural America despite the fact that nearly
			 one-fourth of the population lives in these areas.</text>
			</paragraph><paragraph id="HE2C13DB24FE24E408A4CC187585155DF"><enum>(17)</enum><text>Rural residents are less likely to have employer-provided health care coverage or prescription drug
			 coverage, and the rural poor are less likely to be covered by Medicaid
			 benefits than their urban counterparts.</text>
			</paragraph><paragraph id="H3A1127A01FBF4662BCE991DE1EF07F33"><enum>(18)</enum><text>Twenty percent of nonmetropolitan counties lack mental health services versus 5 percent of
			 metropolitan counties.</text>
			</paragraph><paragraph id="HC45BB48BF3C846F99DA5FF11DAFD285B"><enum>(19)</enum><text display-inline="yes-display-inline">Fifteen percent of persons with disabilities report not seeing a doctor due to cost in comparison
			 to 6 percent of the general population.</text>
			</paragraph></section><section id="H2D0F61CE8F4A4632BC3BCDEAFF19C83E"><enum>3.</enum><header>Institute of Medicine study</header>
			<subsection id="H2914329ADD6B40ACACE7F9A0A2D6184C"><enum>(a)</enum><header>In general</header><text display-inline="yes-display-inline">Not later than 60 days after the date of the enactment of this Act, the Secretary <italic></italic>shall enter into an arrangement with the Institute of Medicine under which the Institute agrees to
			 study—</text>
				<paragraph id="H84B9CD30EA9A4651AFA67F10A606A6DC"><enum>(1)</enum><text>the extent of health disparities in the type and quality of preventive interventions, health
			 services, and outcomes in the United States;</text>
				</paragraph><paragraph id="H4622A7AC99EF45C287A2A52A9FD67B18"><enum>(2)</enum><text>the factors that may contribute to inequities in such disparities;</text>
				</paragraph><paragraph id="H90AE36FF6BC4443590F7008306FE5880"><enum>(3)</enum><text>existing programs and policies intended to reduce such disparities;</text>
				</paragraph><paragraph id="H2424B66AFF694677BD1404555B04B5AC"><enum>(4)</enum><text>best practices and successful strategies in programs that aim to reduce such disparities;</text>
				</paragraph><paragraph id="HDC5D18A4477C41C18BB4164BDB687A8D"><enum>(5)</enum><text>priorities for successful intervention programs targeting such disparities; and</text>
				</paragraph><paragraph id="H56E54E80DD3B4DAAB6A9BC4E52137D67"><enum>(6)</enum><text>potential opportunities for expanding or replicating such programs.</text>
				</paragraph></subsection><subsection id="HC4678D983B8740309134C4A0342E643F"><enum>(b)</enum><header>Report</header><text>The arrangement under subsection (a) shall provide for submission by the Institute of Medicine to
			 the Secretary and Congress, not later than 20 months after the date of
			 enactment of this Act, of a report on the results of the study.</text>
			</subsection></section><section id="H5DA03992EBED4C13BD10D470DB764696"><enum>4.</enum><header>Guidelines for development and implementation of Health Disparities Reduction Programs and
			 Activities</header>
			<subsection id="H23820B39A2DD45709F7CDC47B31CA4A6"><enum>(a)</enum><header>Guidelines</header><text display-inline="yes-display-inline">Not later than 90 days after the submission of the report described in section 3(b), and taking
			 such report into consideration, the Secretary shall develop guidelines for
			 entities to develop and implement programs and activities to reduce health
			 disparities.</text>
			</subsection><subsection id="H52E6A0FACD2240FFB2F8C1730FE307DE"><enum>(b)</enum><header>Use by HHS</header><text display-inline="yes-display-inline">The Secretary shall, where appropriate, incorporate the use of the guidelines developed under
			 subsection (a) into the programs and activities of the Department of
			 Health and Human Services.</text>
			</subsection><subsection id="HE8E4FB66E0D443DBB7DBBCD05D055909"><enum>(c)</enum><header>Grants for disparities reduction activities</header>
				<paragraph id="HA5CE8D19965A4240BE0A811CF0E4DBE0"><enum>(1)</enum><header>In general</header><text display-inline="yes-display-inline">The Secretary may award grants to entities for the development and implementation of programs and
			 activities to reduce health disparities in accordance with the guidelines
			 described in subparagraph (a).</text>
				</paragraph><paragraph id="H08C8A8074C754C2AAC48EC48275A78B3"><enum>(2)</enum><header>Applications</header><text>To seek a grant under this subsection, an entity shall submit an application to the Secretary at
			 such time, in such manner, and containing such information as the
			 Secretary may require.</text>
				</paragraph><paragraph id="H68371B32CA6E4DFBA4CEB72404A0AAAC"><enum>(3)</enum><header>Minimum contents</header><text display-inline="yes-display-inline">The Secretary shall require that an application for a grant under this subsection contains at a
			 minimum—</text>
					<subparagraph id="H9814596A96E74D3E9B228A4366C13716"><enum>(A)</enum><text display-inline="yes-display-inline">a description of the population and public health concern the program will target and an outreach
			 plan to ensure that the most in need populations will benefit;</text>
					</subparagraph><subparagraph id="H53451D0015C641F68C1FA4B64368B01E"><enum>(B)</enum><text display-inline="yes-display-inline">a description of the strategies the entity will use—</text>
						<clause id="HE9F655B635A44DCCB7CDCE566697A0DA"><enum>(i)</enum><text>to develop and implement its programs and activities in accordance with the guidelines developed
			 under subsection (a); and</text>
						</clause><clause id="HD861F722793541C4969E12F18D7C38DB"><enum>(ii)</enum><text>to make the interventions sustainable; and</text>
						</clause></subparagraph><subparagraph id="H114337C24C5B47C8A960755B727397E6"><enum>(C)</enum><text display-inline="yes-display-inline">an agreement by the entities to periodically provide data with respect to—</text>
						<clause id="H5F29E795128C49A29B9D346971A0FA2E"><enum>(i)</enum><text>the population served;</text>
						</clause><clause id="HF44B94F1A35B43A9ADE0B821324AF382"><enum>(ii)</enum><text>improvements in reducing health disparities; and</text>
						</clause><clause id="H54C8C2F91BA5486D871CA12308D130AF"><enum>(iii)</enum><text>effectiveness of the interventions used.</text>
						</clause></subparagraph></paragraph></subsection><subsection id="H4F1E58AA49DB416A972AD1E63DBBE281"><enum>(d)</enum><header>Appropriations</header><text display-inline="yes-display-inline">To carry out this section, there are authorized to be appropriated $5,000,000 for fiscal year 2016
			 and such sums as may be necessary for each of fiscal years 2017 through
			 2020.</text>
			</subsection></section><section commented="no" id="HE71A815482494B3FA3D4C8AA49FF9D4D"><enum>5.</enum><header>Testing alternative payment and delivery models to reduce health disparities</header>
			<subsection commented="no" id="H72073B3EE3794BF3B72477294D487195"><enum>(a)</enum><header>In general</header><text>The Secretary acting through the Centers for Medicare and Medicaid Innovation under section 1115A
			 of the Social Security Act (<external-xref legal-doc="usc" parsable-cite="usc/42/1315a">42 U.S.C. 1315a</external-xref>) shall provide for the testing
			 of a payment and service delivery model that includes incentives for
			 reducing health disparities consistent with the cost and quality criteria
			 otherwise applicable to the testing of models under such section.</text>
			</subsection><subsection commented="no" id="H9F9947A729144562AC38E37216D939FD"><enum>(b)</enum><header>Documentation requirement for model testing</header><text>In carrying out subsection (a), the Secretary shall require that an application to conduct such
			 testing of such a model include at least—</text>
				<paragraph commented="no" id="H014BCB698A8A485EAE3C5A7F69BF6523"><enum>(1)</enum><text>documentation of at least one health disparity targeted for reduction;</text>
				</paragraph><paragraph commented="no" id="H51D449C6C1824D88BBD582FBD2555AE2"><enum>(2)</enum><text>a root-cause analysis of the health disparity targeted for reduction;</text>
				</paragraph><paragraph commented="no" id="H15F35C4A00604D84A03254D43D7B31EB"><enum>(3)</enum><text>identification and selection of performance targets for such reduction;</text>
				</paragraph><paragraph commented="no" id="H3F6CA2D61A1548F79F7BE53F6C4CF840"><enum>(4)</enum><text>a proposal to make payments in some way contingent on a reduction in health disparities; and</text>
				</paragraph><paragraph commented="no" id="H7F18034C93604BB3854DBAFDDA5D1B22"><enum>(5)</enum><text>a reliable method for monitoring progress in achieving such a reduction.</text>
				</paragraph></subsection></section><section id="H494AD140E5794BE898827CE308E59F86"><enum>6.</enum><header>Definitions</header><text display-inline="no-display-inline">In this Act:</text>
			<paragraph id="HAF0C40AA56844D59BA956F2811B8DA10"><enum>(1)</enum><text>The term <term>health disparity</term> means significant disparity in the overall rate of disease incidence, prevalence, morbidity,
			 mortality, or survival rates in a population as compared to the health
			 status of the general population.</text>
			</paragraph><paragraph id="HE2D19ACBA7EE44ED834B2D76379FA2B3"><enum>(2)</enum><text>The term <term>intervention</term> means an activity taken by an entity on behalf of individuals or populations to reduce health
			 disparities.</text>
			</paragraph><paragraph id="H69086337CDC94E39AE5C776A47BCA22B"><enum>(3)</enum><text>The term <term>Secretary</term> means the Secretary of Health and Human Services.</text>
