		<official-title>To amend the Older Americans Act of 1965 to define care coordination, include care coordination as
			 a fully restorative service, and detail the care coordination functions of
			 the Assistant Secretary, and for other purposes.</official-title>

		<section id="H7AFB458F76194D20B1762CB965B1568F" section-type="section-one"><enum>1.</enum><header>Short title</header><text display-inline="no-display-inline">This Act may be cited as the <quote><short-title>Care Coordination for Older Americans Act of 2014</short-title></quote>.</text>
		</section><section id="HF0A336AE57F14557B28168A4CE595649"><enum>2.</enum><header>Declaration of objectives</header><text display-inline="no-display-inline">Section 101(4) of the Older Americans Act of 1965 (<external-xref legal-doc="usc" parsable-cite="usc/42/3001">42 U.S.C. 3001(4)</external-xref>) is amended by inserting <quote>care coordination and</quote> after <quote>including</quote>.</text>
		</section><section id="H409C53E3065D41F48E844BFADD778100"><enum>3.</enum><header>Definitions</header><text display-inline="no-display-inline">Section 102 of the Older Americans Act of 1965 (<external-xref legal-doc="usc" parsable-cite="usc/42/3002">42 U.S.C. 3002</external-xref>) is amended by adding at the end the
			 following:</text>

				<paragraph commented="no" display-inline="no-display-inline" id="H633CF745C9424976B5F7E8FA05E7C969"><enum>(55)</enum>
					<subparagraph commented="no" display-inline="yes-display-inline" id="HAE1F96A166F34BC78A9BE136FC3D6246"><enum>(A)</enum><text display-inline="yes-display-inline">The term <term>care coordination</term> means a person- and family-centered, assessment-based, and interdisciplinary approach to meet the
			 needs and preferences of an older individual and a family caregiver while
			 enhancing the capabilities of the older individual (including the ability
			 to self-direct services).</text>
					</subparagraph><subparagraph commented="no" display-inline="no-display-inline" id="H3DB9D891FEF14227951EC2DBC92367FD" indent="up1"><enum>(B)</enum><text display-inline="yes-display-inline">The term <term>care coordination</term> means coordination that—</text>
						<clause commented="no" display-inline="no-display-inline" id="HBFAA223FB315413E9D522AF4344AE512"><enum>(i)</enum><text display-inline="yes-display-inline">integrates health care, long-term services and supports, housing, and social support services in a
			 high-quality and cost-effective manner in which an individual’s needs,
			 preferences, and capabilities are assessed, along with the needs and
			 preferences of a family caregiver;</text>
						</clause><clause commented="no" display-inline="no-display-inline" id="H04D33DDBD22849FF86DD87E9833A4B1F"><enum>(ii)</enum><text display-inline="yes-display-inline">includes, as a core element, the active involvement of the older individual, the family, or a
			 representative appointed by the older individual or legally acting on the
			 individual’s behalf, community-based service professionals, and health
			 care professionals providing care to the older individual, in the design
			 and implementation of an individualized, individual-centered service and
			 support plan, through which the services and supports will be provided in
			 a manner free from conflicts of interest;</text>
						</clause><clause commented="no" display-inline="no-display-inline" id="H3CFB065055D1408089FA130E4754C41E"><enum>(iii)</enum><text display-inline="yes-display-inline">integrates services and interventions that are implemented, monitored, and evaluated for
			 effectiveness using an evidence-based process, which typically involves a
			 designated lead care coordinator and involves feedback from the older
			 individual;</text>
						</clause><clause commented="no" display-inline="no-display-inline" id="HEB95BCD6FC954A188051BAF0FE312C0D"><enum>(iv)</enum><text display-inline="yes-display-inline">includes activities that aim simultaneously at meeting individual and family needs and preferences,
			 building on individual capabilities, and improving outcomes and systems of
			 care;</text>
						</clause><clause id="H4A54DE332ACF43CF8586F2405AE069AA"><enum>(v)</enum><text>includes provision of some or all of the services and activities described in clauses (i) through
			 (iv) by trained professionals employed by or under a contract with—</text>
							<subclause id="HF7600F6FA1BF4D90B5A996DBE5D3F1B6"><enum>(I)</enum><text>area agencies on aging;</text>
							</subclause><subclause id="HD4F1C1572B164919A62349708E7E8ED5"><enum>(II)</enum><text>Aging and Disability Resource Centers; or</text>
							</subclause><subclause id="H7B676BBABEEE445484AA162327F6D4D7"><enum>(III)</enum><text>other service providers, including in-home service providers; and</text>
							</subclause></clause><clause id="H983ED22DC5554220AC8430690EB6684A"><enum>(vi)</enum><text>is not furnished to directly diagnose, treat, or cure a medical disease or condition.</text></clause></subparagraph></paragraph><after-quoted-block>.</after-quoted-block></quoted-block>
		</section><section id="H0B9924E0C487499A8481A14CB5A15A82"><enum>4.</enum><header>Functions of the Assistant Secretary</header><text display-inline="no-display-inline">Section 202(a) of the Older Americans Act of 1965 (<external-xref legal-doc="usc" parsable-cite="usc/42/3012">42 U.S.C. 3012(a)</external-xref>) is amended—</text>
			<paragraph id="H4BFD999E04DA47A59EADD23194ABED7C"><enum>(1)</enum><text display-inline="yes-display-inline">in paragraph (27), by striking <quote>and</quote> at the end;</text>
			</paragraph><paragraph id="HFC12637C6F634CADB4061785EBAFC188"><enum>(2)</enum><text display-inline="yes-display-inline">in paragraph (28), by striking the period and inserting <quote>; and</quote>; and</text>
			</paragraph><paragraph id="H61C8DF573F6545D784668AEEBEAED03A"><enum>(3)</enum><text>by adding at the end the following:</text>
				<quoted-block display-inline="no-display-inline" id="H4AC6BE72BAE74183AF9BB238E297DB4F" style="OLC">
					<paragraph id="H78A1A5DA0BD04497A5B9C19A1CAB174E"><enum>(29)</enum>
						<subparagraph commented="no" display-inline="yes-display-inline" id="HD5613D1BC2BC4AEC85BC5DFC36DD6048"><enum>(A)</enum><text>encourage, provide technical assistance to, and share best practices with, States, area agencies on
			 aging, Aging and Disability Resource Centers, and service providers to
			 carry out outreach and coordinate activities with health care entities in
			 order to assure better care coordination for individuals with multiple
			 chronic illnesses; and</text>
						</subparagraph><subparagraph id="H1460F26CC52546B883FDF7A116209DB9" indent="up1"><enum>(B)</enum><text>coordinate activities with other Federal agencies that are working to improve care coordination and
			 developing new models and best practices.</text></subparagraph></paragraph><after-quoted-block>.</after-quoted-block></quoted-block>
			</paragraph></section><section id="H72D68D6C8B6D454FBE9893E2899981BC"><enum>5.</enum><header>Organization</header><text display-inline="no-display-inline">Section 305(a) of the Older Americans Act of 1965 (<external-xref legal-doc="usc" parsable-cite="usc/42/3025">42 U.S.C. 3025(g)</external-xref>) is amended—</text>
			<paragraph id="HFBB6CEAF189F4ACEBCF173BE4285B3CA"><enum>(1)</enum><text>in paragraph (2), by striking <quote>and</quote> at the end;</text>
			</paragraph><paragraph id="H53C4F9E29A054D64B5E4E30CF099207D"><enum>(2)</enum><text display-inline="yes-display-inline">in paragraph (3), by striking the period and inserting <quote>; and</quote>; and</text>
			</paragraph><paragraph commented="no" display-inline="no-display-inline" id="H3AE7682A97BD43F1849E647C2BC620BF"><enum>(3)</enum><text>by adding at the end the following:</text>
				<quoted-block display-inline="no-display-inline" id="HD29748D252A04635989B7852B88CB90E" style="OLC">
					<paragraph id="HD0B618EC65C145658778774FC96871C9"><enum>(4)</enum><text>the State agency shall promote the development and implementation of a State system to address the
			 care coordination needs of older individuals with multiple chronic
			 illnesses, and shall work with acute care providers, area agencies on
			 aging, service providers, and Federal agencies to ensure that the system
			 uses best practices and is evaluated on its provision of care
			 coordination.</text></paragraph><after-quoted-block>.</after-quoted-block></quoted-block>
			</paragraph></section><section id="H75C2BEBF6C13451595304DAB9BCF07E0"><enum>6.</enum><header>Area plans</header><text display-inline="no-display-inline">Section 306(a) of the Older Americans Act of 1965 (<external-xref legal-doc="usc" parsable-cite="usc/42/3026">42 U.S.C. 3026(a)</external-xref>) is amended—</text>
			<paragraph id="H7BABA5831D834FFAB2049BDE4B451B20"><enum>(1)</enum><text>in paragraph (4)(B)(i)(VII) by inserting <quote>with multiple chronic illnesses or</quote> after <quote>older individuals</quote>;</text>
			</paragraph><paragraph id="HD3F7E23CB7074E1C8D027B79511D405B"><enum>(2)</enum><text display-inline="yes-display-inline">in paragraph (6)(D), by inserting <quote>(including acute care providers)</quote> after <quote>service providers</quote>;</text>
			</paragraph><paragraph id="H8A8309BF593C416C801DA7D5A0ADB6BE"><enum>(3)</enum><text>in paragraph (16), by striking <quote>and</quote> at the end;</text>
			</paragraph><paragraph id="H0CB83E724DE047958F5880BF9198AF93"><enum>(4)</enum><text display-inline="yes-display-inline">in paragraph (17) by striking the period and inserting <quote>; and</quote>; and</text>
			</paragraph><paragraph commented="no" display-inline="no-display-inline" id="HC6C37B6D02C24783B62FB1917D1F7F32"><enum>(5)</enum><text>by adding at the end the following:</text>
				<quoted-block display-inline="no-display-inline" id="H06823F871981402E9229F6C250D253BF" style="OLC">
					<paragraph id="H8964569CD5F4410AB3FD8BB6E0774E8D"><enum>(18)</enum><text>provide assurances that the area agency on aging will—</text>
						<subparagraph id="HC8C4A809D25846FE9B82D0B79C418F8C"><enum>(A)</enum><text>identify existing (as of the date of submission of the plan) care coordination programs and
			 systems;</text>
						</subparagraph><subparagraph id="HD33D5EC22A2D483284DAE47417E21A19"><enum>(B)</enum><text>identify unmet community need for care coordination;</text>
						</subparagraph><subparagraph id="H133F2795957F47B3BF8AFCB3249E02D1"><enum>(C)</enum><text>facilitate the development and implementation of an area-wide system to address the care
			 coordination needs of older individuals with multiple chronic illnesses;
			 and</text>
						</subparagraph><subparagraph id="H97F7F3C598E84529ACCE6376E8345D40"><enum>(D)</enum><text>work with acute care providers, service providers, and Federal and State agencies to ensure that
			 the system uses best practices in its provision of care coordination.</text></subparagraph></paragraph><after-quoted-block>.</after-quoted-block></quoted-block>
			</paragraph></section><section id="HEB6B8ACA444949A39F0F3C49EDA1F134"><enum>7.</enum><header>State plans</header><text display-inline="no-display-inline">Section 307(a) of the Older Americans Act of 1965 (<external-xref legal-doc="usc" parsable-cite="usc/42/3027">42 U.S.C. 3027(a)</external-xref>) is amended—</text>
			<paragraph id="H2699DE15E09B495F895492B3CCD9E2CF"><enum>(1)</enum><text display-inline="yes-display-inline">in paragraph (2)(A), by inserting <quote>care coordination,</quote> after <quote>information and assistance,</quote>;</text>
			</paragraph><paragraph id="HF488DF0BB9CB432584B6773D852B32CE"><enum>(2)</enum><text>in paragraph (17), by striking <quote>and develop collaborative programs, where appropriate,</quote> and inserting <quote>, ensure care coordination, and (where appropriate) develop collaborative programs,</quote>;</text>
			</paragraph><paragraph id="H0C70249682DA43D7AE1F58047B0FA54C"><enum>(3)</enum><text>in paragraph (18), in the matter preceding subparagraph (A), by inserting <quote>and ensure care coordination that integrates long-term care services and other care services,</quote> before <quote>for older</quote>;</text>
			</paragraph><paragraph id="H0EF444433A4945F2BBA6E0E8A0FDC0F2"><enum>(4)</enum><text>in paragraph (23), by striking <quote>with other State services</quote> and inserting <quote>with other Federal and State health care programs and services</quote>; and</text>
			</paragraph><paragraph id="H20A313336F174A5296A89B43CED8DFD3"><enum>(5)</enum><text>by adding at the end the following:</text>
				<quoted-block display-inline="no-display-inline" id="HFCEE818EEB7F4606824095D131E0764B" style="OLC">
					<paragraph id="H202EB580003144E8824C009A588915D5"><enum>(31)</enum><text>The plan shall provide assurances that the area agencies on aging in the State will facilitate the
			 area-wide development and implementation of an area-wide system to address
			 the care coordination needs of older individuals with multiple chronic
			 illnesses, and work with acute care providers, service providers, and
			 other Federal and State agencies to ensure that the system uses best
			 practices and is evaluated on its provision of care coordination.</text></paragraph><after-quoted-block>.</after-quoted-block></quoted-block>
